The SWAT paramedic transports high risk non-ICU in-patients to any temporary clinical setting in a manner that maintains patient safety, ensures adequate communication, supports continuity of patient care and maintains patient comfort.
Provide short-term care and transport of an inpatient during intra-hospital transport for diagnostic tests and procedures.
Characteristic duties and responsibilities include but are not limited to:
- Observe, monitor and document patient condition, including vital signs and report changes to the medically responsible health care professional.
- Measures, reports, and records vital signs, manually and per monitor (vital signs are to be taken as prescribed or by protocol).
- Informs responsible nurse immediately of abnormal findings.
- Must be able to identify the following arrhythmias: Asystole, Ventricular fibrillation, Ventricular tachycardia, Tachy-arrhythmias, Brady-arrhythmias ,Artifact, and Multiple PVCs
- Document findings on the SWAT nursing flow sheet and centricity as outlined in the Documentation Policy.
- Provide for the safety of self, patients and others by adherence to the hospital?s safety guidelines, policies, and practices.
- Uses equipment correctly, per unit policy.
- Clean equipment and work area.
- Transport, ambulate, move and lift patients.
- Assist in positioning patients for procedures, i.e., MRI, CT scanners, ultrasound, etc.
- Provide comfort measures for patients to include provision of blankets, hot/cold packs, pillows, etc.
Is also responsible for:
- Receiving and carrying out transport assignments as needed accurately and precisely while ensuring the standard of patient care and unit operations. All patient care activities will be per Swat policies or delegated and supervised by the SWAT RN, and SWAT administration.
- Communicating observations/concerns regarding a patients condition to the responsible RN.
- Documentation of interventions completed and patient reactions during care.
- Requesting assistance/clarification in the event of unfamiliarity with assigned tasks.
- Participating in continuous quality improvement by involvement in teams as appropriate and by supporting changes recommended through the continuous quality improvement process.
May perform the following only after completion of competency based orientation/evaluation:
- One-touch glucose monitoring
- Venipuncture as directed
- IV insertion and initiation of IV fluid therapy, .9 Normal Saline only, without medication additives as directed.
- Apply c-spine immobilization including rigid cervical collar and backboard
- Monitor stable patients during diagnostic procedures, i.e., CT scan, plain films, angiography
- Transport hemodynamically stable patients with IV antibiotic, heparin drip infusion and other medication infusions per protocol.
- Transport patients receiving blood products after 30 minutes have passed since start of transfusion and two sets of vital signs have been taken, reviewed by the RN, and within acceptable parameters.
- Transport stable ventilator patients.
- Communicates effectively with patient and families.
- Introduces self to patients and families.
- Communicates promptly and accurately with Survival Flight Communication Specialists and other ancillary staff.
- Demonstrates polite telephone etiquette.
- Communicates effectively with hospital staff.
- Provides appropriate information to primary nurse, nurse manager, and/or other staff.
Is restricted from:
- Accepting or relaying verbal physician orders.
- Initiating written orders without RN delegation
- Preparing medication or administering it by any route, including contrast dye.
- Independently performing initial patient assessment.
- Discharge teaching.
- NG tube insertion.
- Obtaining blood specimen from an arterial line except closed systems.
- Accessing venous access devices (i.e., Hickman, Broviac).
- Administration of blood or blood products. Transport patients receiving blood products less than 30 minutes since start of transfusion, or vital signs that are outside acceptable parameters.
- Works with SWAT RN preceptor and clinical supervisor to mutually identify short-term goals for successful completion of orientation and evaluates goals annually.
- Demonstrates an understanding of own learning and developmental need by attending pertinent inservices and other activities.
- Attends BLS, safety, and infection control and all designated mandatory educational activities annually.
- Maintains current ACLS, PALS, and NRP certifications.
- Administrative supervision is received from the Survival Flight / SWAT Nurse Manager and Nursing Supervisor.
- Functional supervision is received from the SWAT Charge Nurse and / or SWAT Registered Nurse on assigned shift during SWAT RN hours of coverage.
- Functional supervision is received from the Survival Flight Nurse or Unit Registered Nurse during hours of no coverage by SWAT RN.
- Current Michigan Licensure as an EMT-P.
- Two (2) years recent (within one (1) year) experience as a Paramedic.
- Excellent interpersonal communication skills.
- Demonstrated leadership skills as evidenced by progressive responsibility in ED / pre-hospital setting.
- Excellent clinical skills including but not limited to IV insertion, veni-puncture, and arrhythmia recognition.
- Excellent organizational skills.
- Demonstrated ability to work effectively under pressure of emergency situations.
- Current Basic Life Support Provider verification.
- Able to make a minimum of one (1) year commitment to SWAT program
- Advanced Cardiac Life Support Provider (ACLS).
- Clinical pediatric experience.
