Through the Guest Assistance Program (GAP), social workers serve as ambassadors/liaisons to the entire Social Work Department and to UMHS patients and families in general.
This position requires basic psychosocial assessment skills to evaluate eligibility for tangible resources, as well as ability to evaluate needs for referral and intervention for patients and families. This position demands the ability to work well with a diverse population in need of a variety of support and services. The ability to conduct brief assessment of psychosocial and tangible needs is essential, as well as the ability to multitask and prioritize on a constant basis. Experience as a contributing member of a successful team is desirable.
GAP social workers provide support by assisting with information and referral, pharmacy and limited financial assistance, transportation, lodging, community resources, wig bank and other tangible needs to facilitate continuity of patient care, as well as access to care.
Bachelor's degree in social work from a school accredited by the Council of Social Work Education (CSWE) is necessary. Must be licensed to practice social work in State of Michigan.
Excellent interpersonal and written communication skills are critical for success in this environment. Word, Access and Excel are utilized on a daily basis. Creating an exceptional patient/family experience is our highest commitment as this role is often the first contact with the UMH Department of Social Work.
Knowledge of local and statewide community resources is desirable, as well as experience as a contributing member of a successful team. Previous experience in customer service and/or healthcare or social service agencies will be particularly helpful for success in this role.
Work Schedule is Monday through Friday 8:30am to 5pm.
