Responsibilities*

Patient Care
Utilize the nursing process to assess the physical status of identified patients in the clinical setting.
Counsel patients/significant others regarding etiologies, diagnostic testing and management of heart failure.
Provide telephone counseling/triage for patients as indicated. Document per policies.
Interdisciplinary Patient Care
Triage and correct signs and symptoms of heart failure in collaboration of medical treatment plan.
Teach and reinforce understanding of self-care goals established by the Heart Failure Management Program through intensive individual education sessions.
Function as a resource to insure continuity of care
Education
Collaborate with others, as needed, to assess and meet educational needs of colleagues.
Plan, design, and coordinate workshops, formal classes, conferences, and/or consultation services as indicated.
Management of the Program
Participates in the development, modification and interpretation of policies and procedures.
Participate in data collection and analysis about patient care and workload, as requested.
Research/Quality Assurance
Initiate and/or participate in data collection and assist in analysis of research project related to patients as indicated.
Professional Development
Attend appropriate continuing education, staff development programs, conferences and grand rounds as indicated.

Salary & Nursing Framework Level:
This UMPNC RN posting is posted as Nursing Framework LEVEL C (Includes B).
Actual Nursing Framework LEVEL and salary will be determined at time of hire.
Nursing Framework levels range from Level A to Level E.

Required Qualifications*

Current license to practice nursing in Michigan is necessary.

Minimum of five (5) years current experience as a registered nurse in cardiology required, with at least three (3) years experience in step down, ICU or ER.

Must be able to travel to off site UM Clinics as needed

Demonstrated competence with computer functions including Excel, Word and two (2) or more years experience with electronic charting.

Demonstrates expert assessment skills in evaluating and treating end-stage Heart Failure patients with multiple co-morbidities, and knowledge of interventions required to prevent     acute exacerbation and hospital admissions.

Demonstrates expert knowledge of adult cardiac physiology.

Demonstrates ability to apply evidence based medical management, and independently utilize protocols to manage care

Demonstrates experience with and knowledge of current medical, interventional and pharmaceutical treatment modalities of patients with Heart Failure.

Demonstrates understanding of complex medication interactions with medications typically given to the adult heart failure patient.

Demonstrates excellent verbal and written communication skills, organization and problem solving skills.

Ability to function independently and as a team member.

Demonstrates ability to communicate assertively as needed as a patient advocate.

Demonstrates high level of critical thinking.

Demonstrates high level of self-motivation to learn, to stay current in the rapid changing treatment of cardiac patients, and to participate in activities to improve patient care.

Demonstrates ability to positively contribute to a group/clinic work setting.

Demonstrates knowledge of the evaluation and treatment of lab abnormalities.

Demonstrates competence in EKG-interpretation for abnormal rhythms.

Desired Qualifications*

Bachelors Degree in Nursing or an equivalent combination of education and experience.

Demonstrates competence in assessing, treating, and evaluating patients' response to treatment of the complex heart failure patient population in an ambulatory care setting.

Demonstrates competence in coordinating care and multi-tasking.

Experience in extreme flexibility in daily operations; ability to prioritize and re-prioritize multitude of daily calls

Demonstrates knowledge and experience with Continuous Quality Improvement.

Ability to deal with high call volume and constant triaging to prioritize work.

Demonstrates active participation on unit activities including but not limited to Professional Practice Councils, other committee or QI work.

Work Schedule

Hours: 40 hours

Shift: Day shift,  Full-time- 8:00am-5:00pm- **Must have the ability to travel to off-site clinics when needed
