Job Summary

Dual Unit Float RN
Purpose of Position:
Provide float support coverage to team members in the areas of hepatology, pre/post adult kidney/pancreas transplant and Pre/Post Liver, with the potential for support to the Transplant Procedure unit if applicable.  This float coverage includes but not limited to case coordination through patient evaluation, assessment, and implementation of planned interventions. Facilitate extensive patient education through telephone intervention and face-to-face meetings in both individual and group settings.  Work collaboratively with the healthcare team to provide quality care in the outpatient setting for hepatology and abdominal transplant population.  
Organizational Relationships:
This staff member is under the direction of the Transplant Ambulatory Care Manager, and will provide Ambulatory Care Nursing support to a variety of nursing units within the Transplant Center.  


Utilize the nursing process to facilitate patient care for chronic disease management as well as through the pre- and post- transplantation stages.
Provide skilled observations, telephone triage and health assessments of patients including history, appropriate interventions, and referrals.
Foster continuity of care and facilitate transitions across outpatient and community patient records on patient population.
Provide consultation and participate in problem-solving efforts with nursing staff, medical staff, referring physicians and other disciplines.
May be involved in the care of Post TXP biopsy and infusion patients.
Maintain knowledge of all applicable regulatory laws and requirements, including HIPAA, CMS Conditions of Participation, The Joint Commission, and UNOS to ensure regulatory compliance in nursing practice.
Foster continuity of care and facilitate transitions across outpatient and community settings.
o Provide continuity through preadmission and/or post discharge outpatient visits, including telephone triage and counseling.
o Provide follow-up on discharge planning with Continuing Care Coordinator, other multi-disciplinary staff and resources in the community.
o Provide continuity through handling of post discharge outpatient care needs prior to next visit.
o Provide instruction/reinforcement regarding self care to increase adherence to prescribed treatment.
Utilize various resources available within the Ambulatory Care and Department of Internal Medicine/hepatology/nephrology transplant settings to determine and meet the needs of the patient/family.
Develop collaborative mechanisms with appropriate others (nurse managers, peers, etc.) for providing patient care.
Collaborate with Nurse Manager, education coordinator and other health team members on units providing care to patients to identify and support educational needs.
Serve as a consultant to nursing personnel and other health care professionals caring for transplant patients.
Represent the transplant team to external agencies, associations and programs, as appropriate.
Initiate, with the interdisciplinary team, evaluation of team process and function, as appropriate.  
Participate in the development, modification and interpretation of policies and procedures, as needed.
Participate in area nursing staff meetings as well as other departmental meetings.
Facilitate timely communications to interdisciplinary staff regarding patient needs/plan of care.  Include referring physicians as appropriate.
Participate in on-going transplant evaluation meetings and the interdisciplinary team process.
Research/Quality Assurance:
Monitor nursing care provided and utilize results of quality monitoring to initiate changes in practices, as indicated.
Participate in continuous quality improvement by involvement in teams and by supporting changes recommended through the continuous quality improvement process.
Participate in medical/multidisciplinary research as requested by
o Identifying research subjects.
o Initiating/participating in data collection/analysis.
o Providing leadership for nursing’s participation in research studies.
Evaluate, interpret and integrate research findings into clinical practice.
Effectively analyze and prioritize outpatient/service needs.
Demonstrate effective time management skills.
Demonstrates flexibility with assignments and tasks.
Demonstrate effective delegation techniques.
Identify opportunities to implement strategies/make recommendations for cost effective care, when appropriate.
Demonstrate problem-solving skills as related to operational and clinical issues within the clinical area and through meeting participation and attendance.
Professional Development:
Assume responsibility for on-going professional development, attending appropriate continuing education, staff development programs, conferences and grand rounds as indicated.
Maintain current knowledge of nursing and professional literature.
Participate with nurse manager in annual evaluation of own performance.
Assume responsibility for attendance at the hospital’s mandatory continuing education programs annually.

Salary & Nursing Framework Level:
This UMPNC RN posting is posted as Nursing Framework REG NURSE - LEVEL C (B INC).
Actual Nursing Framework LEVEL and salary will be determined at time of hire.
Nursing Framework levels range from Level A to Level E.
Required qualifications must be met by the candidate in order to be interviewed and considered for the position.

Required Qualifications*

Current licensure to practice nursing in the State of Michigan.
Five years of nursing experience as an RN
Demonstrated competency in the care of immunocompromised patients.
Demonstrated competency in the assessment and interventions of laboratory testing, management of complex medical regimens, and patient and family education and counseling.
Demonstrated excellent written and verbal communication skills as evidenced by performance and evaluation.
Demonstrated excellent interpersonal skills as evidenced by performance and evaluation.
Ability to interact collaboratively with all members of the interdisciplinary team effectively and manage multiple tasks.
Demonstrated excellent attendance as evidenced by performance and evaluation.
Critical thinking, as demonstrated in written and verbal communication, in the care of complex patients.
Transplant certification (CCTC/CCTN) within 2 years of hire.

Desired Qualifications*

2-5 Experience in the Ambulatory Care setting.
Experience in the care of  Kidney and Liver Transplant & Hepatology, patients in the Ambulatory Care setting.
Experience in the care of patients with surgical wounds.
Experience in providing care to multiple patient disciplines.
Experience with EPIC, OTIS, Avitrax, Word, Excel.
Knowledge of UMMC nursing policies and standards of practice.
Bachelor of Science Degree in Nursing.

Note: All new employees will be expected to float to a designated unit(s) in times of low census.
