Job Summary

The West Ann Arbor Health Center – Parkland Plaza is the newest Ambulatory Care facility dedicated to providing high quality health care services for adults and children.  The brand new 2-story facility is located 380 Parkland Plaza in Ann Arbor.  We are actively seeking compassionate, enthusiastic, and patient-oriented individuals who will share in the goal of creating the ideal experience for our patients, families and employees.
Anticipated opening of the new West Ann Arbor Health Center – Parkland Plaza is November 2017.
Participates in the planning, implementation and evaluation of nursing care under the direction of a Physician or Registered Nurse.
Related Duties
Perform other related duties as assigned.
Supervision Received 
General supervision is received from a Provider or Registered Nurse.
Physical Requirements
This position requires moderate standing/walking, as well as lifting, positioning, pushing and/or transferring of patients.



* Direct Patient Care: Assists the provider or Registered Nurse (RN) with tests and procedures such as sterile and clean dressings, urinary catheterizations, administration of medications, medical emergency care and other responsibilities.  Alerts provider or RN of significant changes in patient's condition.
* Medication Administration: Prepares and administers medications using the 5 rights and two patient identifiers via a written physician order or by approved protocol.  Documents medications in MiChart in a timely and accurate manner.
* Telephone Management: In person or on the telephone, collects patient information for the RN or provider.  Provides information to patient as instructed by the provider or RN.  Documents patient care and observed responses accurately and completely in MiChart and STORK.
* Clinical Support Processes: Provides accurate and efficient clinical support processes that include prescription refills through use of approved protocols or provider written orders, test results reporting, pharmacy insurance authorization, and /or other requests. Also performs supply inventory and ordering e.g. VFC, medications. 
* Education and Training: Maintains current competencies and mandatory training. Attends mandatory clinic and hospital training programs.  Actively participates in clinic and/or staff meetings.  Assists in staff orientation efforts.

Required Qualifications*

  • Graduation from an approved school for practical nursing is necessary. 
  • Current licensure as a Licensed Practical Nurse in the State of Michigan.  
  • Successful completion of a pharmacology course in an accredited program.  
  • Relates work and job purpose to UMHHC mission and commitment to putting patients and families first. 
  • Good attendance.
  • Demonstrates effective communication that enhances productivity and builds respectful relationships.  Demonstrates active listening, written, verbal, and information technology skills.  Shares relevant information.

Desired Qualifications*

  • Recent (within last 5 years.) Ambulatory Care LPN experience.
  • Recent (within last 5 years) Pediatric and/or OB experience.
  • Knowledge and use of an electronic medical record e.g. MiChart.
  • Proficiency with Microsoft Office.
  • Knowledge of UMHS policies and procedures.

Work Schedule

Hours: 40 hours per week 
Days/Shift: Days/Shift: Monday through Friday;  Day shift; 8-hour shifts; hours and days may vary and are dependent on clinic operational needs.
Location: West Ann Arbor Health Center- Parkland Plaza

Background Screening
