Taubman Center General Medicine Clinic is seeking a full-time (1.0 FTE) social worker to provide care management and psychotherapy services for the diverse population of adult patients served in their primary care clinic setting. The social worker will provide diagnostic and psychosocial assessments, care management and care coordination, face-to-face and telephone contacts with patients and/or their families, patient education and family interventions, brief evidence-based mental health interventions in a health care setting, safety planning, grief counseling, referrals to community resources and some provision of tangibles. Efficient, accurate assessment of patient needs and effective case dispositions are necessary. Excellent communication and organizational skills, collaboration with a multi-disciplinary team, proficiency in using a computer, keeping accurate and timely clinical records, participation in team meetings, and the ability to effectively prioritize multiple competing tasks in a fast-paced clinic are required. It is essential that the candidate be able to handle and provide high-level therapeutic interventions for a large caseload of patients with complex medical and/or psychiatric histories.
- A Master's degree (MSW) in social work from a school accredited by the Council of Social Work Education is necessary.
- Candidates must have a current clinical license (LMSW) to practice social work in the State of Michigan, with clinical certification.
- LLMSWs will not be considered for this position.
The ideal candidate will have significant medical and behavioral social work experience. Strong assessment skills, ability to relate well and to establish therapeutic relationships quickly with a diverse patient population, and the ability to effectively prioritize competing demands and work independently are necessary. This clinic has a large medical residency training program, and the successful social worker will be actively involved in reaching out to educate residents about social work services that assist and support clinic patients. Experience delivering evidence based interventions such as Motivational Interviewing, Behavior Activation, CBT, DBT, ACT, and Solutions Based therapies is desirable. The successful candidate will possess qualities that include flexibility, adaptability, collaboration, excellent communication, and ability to access community resources. Certification by the Academy of Certified Social Workers (ACSW) is desirable.
The work schedule is Monday-Friday from 8:00-4:30 or 8:30-5:00.
This position may be underfilled at a lower classification depending on the qualifications of the selected candidate.
