Job Summary

Responsible for developing and implementing Silver Club memory loss and activity programs for older adults with dementia.

Assigned hours, days of week, and specific work assignments may be modified in response to program, department or service needs.


Collaborate with all Memory Program Specialist Team Members to plan, implement and evaluate a daily dementia-care activity program:

  • Plan and implement a full schedule of functionally and age-appropriate, engaging activities for Silver Club participants, including large and small groups, customized to meet individual needs.
  • Responsible for purchasing, organizing and maintaining program materials, supplies and equipment.
  • Assist with training of and oversee volunteers, student interns and temporary staff assisting in the daily program.
  • Assist individual club members with personal care as needed, including toileting, mobility and changing soiled clothes, washing.
  • Provide a safe and comfortable environment focusing on the well-being of program participants.
  • Set up program rooms/areas including tables, chairs and equipment and monitor for ongoing accessibility/safety.
  • Plan, prepare, serve meals and snacks.
  • Provide appropriate monitoring and supervision of participants during meals and snack-times
  • Provide appropriate monitoring and supervision of participants taking medications.
  • Respond to emergency situations quickly and calmly per agency policies and procedures.
  • Be knowledgeable of participants/ backgrounds, abilities and challenges and monitor continuously.
  • Communicate with participants’ family members about participants’ accomplishments, needs, and changing cognition, health and/or behavior.
  • Monitor participants’ continued appropriateness/eligibility for program, and advise Director of potential discharges. 
  • Monitor daily attendance and recommend immediate staffing adjustments/reassignments.
  • Assist with new participant assessment visits
  • Coordinate participants’ program transportation services.

Administrative duties include:

  • Participate as a Care Planning Team member, developing goals and care guidelines according to the participants’ needs; document and monitor per program standards.
  • Document members’ activities, progress, changes and any other significant events per program standards.
  • Submit required statistics and assist with reporting.
  • Participate in staff meetings and other internal boards/committees as assigned.
  • Assume responsibility for development and implementation of 1-2 key program responsibilities/tasks, such as general volunteer recruitment, coordination and reporting; special student/volunteer service or training projects; developing collaborative arrangements with targeted groups or individuals; special reports.
  • Provide input/assist with development of program policies, procedures, forms and tracking systems.

OTHER DUTIES may include the following:

  • Assist with outreach programs and community presentations
  • Participate in community task forces.
  • Assist with fundraising projects.

Required Qualifications*

  1. Bachelor’s degree in human services or humanities from an accredited school.
  2. Minimum of 1 year experience working in a professional setting  with  older adults, preferably those with memory loss
  3. Physical ability to assist participants with ambulation and transfer.
  4. Physical ability to move/set up tables, chairs, program equipment.

  8. Demonstrated ability to problem-solve and exercise judgment within the scope of position responsibilities.
