Job Summary

The Michigan Medicine Stroke Program is seeking a highly motivated NP/PA to join our busy team in a full-time position. Our program is certified as a Comprehensive Stroke Center with a highly effective leadership team. Our stroke program has seven vascular neurologists who see stroke patients in both the inpatient and outpatient setting, and we have an established telestroke relationship with several affiliate hospitals. This full-time position will consist of

  • Four half-days of outpatient clinic seeing patients who have recently been discharged from the hospital after a stroke
  • Triaging and seeing patients who are referred to our clinic in need of an expedited stroke evaluation
  • Chart review and quality metric extraction related to our growing telestroke operations
  • Facilitation of feedback regarding transfer-in and telestroke patient outcomes to referring hospitals and providers
  • Leadership and coordination of processes related to the stroke inpatient-to-outpatient transition with the specific goals of improving patient satisfaction and reducing re-admission rates


  • This Advanced Practice Provider will provide health care and specialized health services to patients and families enrolled in the Comprehensive Stroke Program in Cardiovascular Medicine at the CVC Clinic.
  • Under physician supervision, the Advanced Practice Provider will coordinate care of clinic patient’s, perform consultations, pre-op evaluation and history, and follow up with the patients.
  • Will also provide clinical expertise and direction to support patient medication management.
  • Will collaborate with other care providers/consultants to coordinate and communicate clinical care
  • Maintain accurate documentation regarding patient status, evaluations, and treatments to physicians, the RN/Clinical Team; streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external Michigan Medicine team members and other ambulatory care providers.
  • Participate in quality assurance activities including Lean Projects with the Comprehensive Stroke clinical team as necessary.
  • Perform other related duties as required to assure patient safety and satisfaction.
  • Participate in the development and presentation of educational experiences for patients, families and health care professionals.
  • Utilize Service Excellence and Patient Family Centered Care in daily practice.
  • Abide by the standards of care and standards of practice established by Michigan Medicine.
  •  Practice within the regulations of the Michigan Public Health Code.
  • Participates in the quality improvement for transitions of care.


Characteristic Duties and Responsibilities


  1. Tele-stroke quality and feedback MidMichigan, Metro, St Joe’s Ann Arbor and Hurley. The APP will generate a spreadsheet and individual case reports for U of M treating physicians to improve best practices and encourage continued participation. Feedback letters for each case will be generated for partner organizations to be sent out by Dr. McDermott.  Metric: The APP will be responsible for 100% of these cases.
  2. Triage and seeing urgent clinic referrals especially from partner organizations. Metric: 50% of urgent stroke clinic referrals will be seen within 72 hours and >90% will be seen within 5 business days.
  3. Coordination of inpatient to outpatient to long-term management of the stroke patient. As we begin to examine the transitions of care process for stroke and anticipate this becoming a major JC indicator, we need to evaluate the needs and available resources for stroke patients who are discharged from University Hospital and return to the community. The goals are to maximize patient satisfaction, reduce ED and re-admissions and comply with anticipated rigorous JC evaluation. The APP will lead the effort to establish resources and unmet needs. Metric: a completed report within 12 months of hire.
  4. Medical/nursing Relationship Manager for stroke partnerships around the State. As the demand for our services expand there is increasing need for a Relationship Manager to record, track and ensure ongoing success in our operations.  Examples include improving the speed and efficiency of transfers from ST Joe’s Ann Arbor to University Hospital for acute endovascular care, and sharing protocols for primary and comprehensive stroke center certification with MidMichigan and Metro. Metric: >80% positive evaluation by U of M and partner organizations on the value and contribution of the APP to these processes.

Required Qualifications*


  • Current license to practice as a Registered Nurse in Michigan.
  • Current specialty certification as a Nurse Practitioner in the State of Michigan.
  • Graduate of Accredited Nurse Practitioner program as an Adult Gerontology Acute or Primary Care NP, Adult NP, or Acute NP 
  • Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
  • DEA license required, or ability to obtain DEA license within 30 days of start date required


  • Current license to practice as a Physician Assistant in the State of Michigan.
  • Certification by the National Commission on Certification for Physician Assistants (NCCPA).
  • Satisfactory completion of a physician assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA), or one of its predecessor agencies, CAHEA or CAAHEP.
  • DEA license required, or ability to obtain DEA license within 30 days of start date required

Required Qualifications-NP/PA:  

  • At least 2-5 years’ recent experience in a leadership role as a Registered Nurse and/or Advanced Practice Provider caring for patients in the emergency room and/or care of stroke population.
  • Minimum of 2 years of patient triage
  • Experience caring for Stroke population
  • Demonstrated ability to work collaboratively and effectively with a multidisciplinary team.   
  • Demonstrated experience with and knowledge of current medical, surgical and pharmaceutical treatment modalities of the post-stroke patients.   
  • Ability to function independently and partner with team members to create ideal patient outcomes.  
  • Demonstrated commitment to patient and family centered care. 
  • Demonstrates desire to work with community groups
  • Participation in Professional organizations/memberships
  • Current ACLS credential.  
  • Demonstrated high level of critical thinking. 
  • Demonstrated high level of motivation to learn and participate in activities to provide outstanding patient care, improve stroke patients experience with the inpatient-to-outpatient transition, and serve the needs of our affiliate hospitals and partnerships.
  • Must reflect a professional appearance and demeanor and possess excellent customer service skills.
  • Demonstrated excellent verbal and written communication skills
