A total of one (1), 1.0 FTE Advanced Practice Professional (either NP or PA) will be hired to fill this job opening.


  • This Advanced Practice Provider (APP) will provide health care and specialized health services to patients and families enrolled in the Heart Failure Program in Cardiovascular Medicine Special Programs
  • Under physician supervision, the APP will coordinate care of this patient population before and after hospitalization
  • See patients in the clinic for initial and return visits as well as pre & post hospitalization appointments.
  • Will also provide clinical expertise and direction to support patient medication management
  • Will collaborate with other care providers/consultants to coordinate and communicate clinical care
  • Maintain accurate documentation regarding patient status, evaluations, and treatments to physicians, the heart failure Tele-management RN/Clinical Team; streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external UMHS team members and other ambulatory care providers
  • Participate in quality assurance activities including Lean Projects with the Heart Failure Tele-management Team
  • Perform other related duties as required to assure patient safety and satisfaction
  • Participate in the development and presentation of educational experiences for patients, families and health care professionals.
  • Utilize Service Excellence and Patient Family Centered Care in daily practice
  • Abide by the standards of care and standards of practice established by University of Michigan Health System 
  • This APP will work directly with multiple Heart Failure physicians to provide care across the continuum 
  •  Practice within the regulations of the Michigan Public Health Code

Required Qualifications*


  • Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
  • Current specialty certification to practice as an Adult-Gerontology Acute Care or Primary Care NP, Adult NP, or Acute NP  
  • Graduate of an accredited Nurse Practitioner program.
  • Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
  • DEA license required, or ability to obtain DEA license within 30 days of start date required


  • Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
  • Certification by the National Commission on Certification for Physician Assistants (NCCPA).
  • Current license to practice as a Physician Assistant in the State of Michigan.
  • DEA license required, or ability to obtain DEA license within 30 days of start date required


  • 3-5 years’ experience in cardiac patient population
  • Current ACLS certification
  • Demonstrated excellent verbal and written communication skills
  • Demonstrated ability to collaborate effectively in a multidisciplinary team
  •  Demonstrated ability to complete history and physical examination; order appropriate diagnostic studies; synthesize information and make informed clinical decisions; develop appropriate care plans; responsive to patient and family preferences and needs
  • Demonstrated high level of critical thinking

Desired Qualifications*

  • 3-5 years’ recent experience as a NP/PA caring for Heart Failure or Heart Transplant patients in an Inpatient setting
  • Previous experience in ambulatory cardiology clinic

Work Schedule

Hours/Week:  40 hours
Day shift: Five- 8 hour shifts Monday through Friday and potential on-call Saturday/Sunday for tele-management
Location: CVC and travel to off-site clinics.
