Responsibilities*

Provide comprehensive case management services to adults as part of an interdisciplinary team which includes a Nurse Practitioner. The ideal candidate will function as an ongoing liaison, ensuring adequate communication between the patient and care providers. Assists patients in achieving optimum stability of health status through service coordination, support, and collaboration with the patient, family members, treating providers, and community agencies. The candidate will assess patients in their homes in multiple dimensions - mental, physical, behavioral, social, functional, and environmental. Implement, coordinate, monitor, and evaluate the options and services required to meet the patient's medical and psychosocial needs in conjunction with the treatment plan developed by the Interdisciplinary team. Ensure coordination of health-care services across a variety of disciplines and settings as patients transfer between different locations or different levels of care. Facilitate patients in scheduling appointments, arrange non-emergent transportation as appropriate and pursue resource development to assist patient with identified barriers. Performs screening and case review of potential cases for assigned patient population. Identifies high-risk and high-cost patients with complex medical and/or psychosocial needs and assesses treatment options and opportunities to improve the quality and efficacy of care.

Required Qualifications*

Bachelor's Degree in Nursing, Social Work, Psychology, or other related clinical field is required. Possess current driver’s license. Two or more years of progressively responsible experience providing care/case management services. Strong knowledge of community resources, governmental-funded programs, and health plans. Strong interpersonal communication skills including problem solving, decision making, and crisis intervention are required. Strong organizational and written and verbal communication skills are required. Strong dedication to customer service and ability to work within a team-focused, participative management framework are required. Case Management certification or ability to become certified within one year of employment. Demonstrated computer competency and ability to type 40-60 words per minute.

Desired Qualifications*

Current licensure or registration is desirable. Experience working with vulnerable adults, with understanding of how to access programs and resources specific to this population.
