Job Summary

Basic Function:  Oversee nutritional care for patients in the clinic setting with a wide range of health conditions.  Patient ages may range from infants to adult.  Support patients in behavior-based goal setting.  Demonstrate flexibility in job role to meet patient, unit and organizational needs.

Organizational relationships

  • Supervision received from Manager of Ambulatory Care Nutrition Services, Ambulatory Care Administrative Leader, with oversight provided by Ambulatory Care Medical Director
  • Liaison relationship with interdisciplinary health care team for patient care populations of responsibility.
  • Peer relationships with Registered Dietitian Nutritionists (RDN) in work group.

Responsibilities*

Patient Care

  • Conduct an ongoing comprehensive nutrition assessment for patients identified at nutritional risk and/or for patients identified to be seen as per ambulatory care clinic standards.
  • Incorporate evidence-based standards when interpreting patient related data.
  • Organize patient health and disease information in order to identify nutrition-related problems.
  • Evaluate need for specialized nutrition support and make appropriate recommendations accordingly.
  • Develop and implement an individualized plan for nutrition intervention. 
  • Perform nutrition diagnosis after identifying the problem and clarifying the cause of the problem.
  • Ground interventions through review of quality research and/or use of medical nutrition therapy evidenced-based practice.
  • Chart nutritional diagnosis, individualized nutrition treatment plan, follow-up documentation of patient outcomes and revisions to plan in the electronic medical record.  This communication is performed in a timely manner that is consistent with unit policy and procedures.
  • Obtain prior authorizations for nutritional supplements and support regimens as needed.
  • Recognize the varied needs of age-specific populations, as well as cultural, religious, and ethnic concerns. 
  • Address patient’s specific needs in the nutrition care plan and honor any advanced directives.
  • Measure effectiveness of nutrition interventions in reaching desired outcomes.
  • Reassess the nutrition care process and implement changes as indicated.
  • Communicate regularly with the medical team through interdisciplinary ambulatory care clinic coverage.
  • Abide by Michigan Medicine policy for the proper use of patients’ protected health information.
  • Abide by Michigan Medicine policy for the proper use of infection prevention precautions in patient care areas.
  • Precept dietetic interns.
  • Participate in nutrition research studies and communicate findings through reports, abstracts, presentations, and publication.

Business Functions

  • Utilize charge capture by RDN for RDN services.
  • Provide quality service based on customer expectations and needs.

  • Maintain productivity standards and practice effective time management.
  • Assist in achieving compliance with state and federal regulatory and accrediting agencies.
  • Participate in interdisciplinary meetings, task forces, and projects as appropriate.
  • Contribute to quality improvement efforts.
  • Collect outcome data to help improve practice.

Required Qualifications*

  • Able to work with a team to accomplish organizational goals.
  • Communicate in a clear, concise and diplomatic manner both verbally and in writing.
  • Implement critical thinking to solve problems.
  • Use technology such as word processing, spreadsheet, and e-mail.
  • Bachelor degree in a recognized field of science or learning which is directly related to the duties of the position.
  • Registered Dietitian Nutritionist with active registration by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics.  Requires primary source verification.
  • Completion of a didactic program in dietetics and supervised practice program approved by ACEND (Accreditation Council for Education in Nutrition and Dietetics) of the Academy of Nutrition and Dietetics.

Desired Qualifications*

  • Master’s Degree in Nutrition related field. 
  • Documented experience working with pediatric and adult patients in the primary care setting.
  • Certified Diabetes Educator (CDE), OR Commission on Dietetic Registration Certificate of Training in Weight Management

Work Schedule

The Registered Dietitian Nutritionist filling this role will be asked to provide services in alignment with the clinic’s evening hours on Tuesdays.
