Responsibilities*

  • Conducting targeted psychosocial assessments to identify and assist with environmental challenges related to medical condition, treatment, hospitalization, and/or readiness to engage in care
  • Identifying barriers to care that interfere with obtaining maximum benefit from health care and/or maintenance of health, such as transportation, prescription costs, lodging, employment concerns, medical equipment, housing, utilities, literacy, and other financial or social service needs
  • Determining benefits and financial assistance eligibility and/or available resources, with specialized skill in working with the un/underinsured
  • Providing appropriate interventions to match patient/family need, including patient education, support, advocacy, and care coordination
  • Serving as a clearinghouse for financial, insurance, and community resource information for staff, patients and families, and outside agencies
  • Providing referral and linkage to appropriate community resources

Required Qualifications*

Excellent interpersonal and written communication skills are critical for success in this environment.  Word, Access, Excel and Epic are utilized on a daily basis.  This position requires brief psychosocial assessment skills to evaluate eligibility for tangible resources, as well as ability to evaluate needs for referral and intervention for patients and families.  This position demands the ability to work well with a diverse population in need of a variety of support and services.

The schedule for this position will be Wednesday-Sunday, 8-hour day shifts.  On weekends, this role will be the sole representative of the GAP, therefore candidates need to be comfortable with fast-paced and independent critical thinking, problem-solving, prioritization and management of workload.
