A total of one(1), 1.0 FTE, Advanced Practice Professional (either NP or PA) will be hired to fill this job opening.

Job Summary

This Advanced Practice Professional (NP/PA) will provide health care and specialized health services to patients and families enrolled in the Trans-Catheter Mitral Value Replace (TMVR)/Tricuspid Program in Cardiovascular Center Clinic.

Responsibilities*

Under physician supervision, the Advanced Practice Professional(NP/PA) will
  • Coordinate care of clinic patient’s, perform consultations, pre-op evaluation and history, and follow up with the patients post-operatively, including but not limited to placing orders and following up on patients testing to ensure complete evaluation for the TMVR Procedure.  
  • Participate in clinical trials now and in future.  
  • Provide Direct Advanced Practice support for the Cardiologist and Cardiac Surgeon visits.
  • Will collaborate with other care providers/consultants to coordinate and communicate clinical care.
  • Provide leadership and innovation to support the growth of the TMVR program.  Strong skill of coordinating a multidisciplinary approach to support the patients in the program. 
  • Round on inpatient setting for new consults and collaborate with inpatient clinical team/Ambulatory CCC to facilitate transitions of care.
  • Will also provide clinical expertise and direction to support patient medication management
  • Chair, organize, and maintain the TMVR Case Presentation meetings biweekly.
  • Maintain accurate documentation regarding patient status, evaluations, and treatments to physicians, the RN/Clinical Team; streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external UMHS team members and other ambulatory care providers
  • Participate in quality assurance activities, process improvement projects (LEAN), and facilitate growth in our Mitral TMVR Program.
  • Perform other related duties as required to assure patient safety and satisfaction
  • Participate in the development and presentation of educational experiences for patients, families and health care professionals.
  • Utilize Service Excellence and Patient Family Centered Care in daily practice
  • Abide by the standards of care and standards of practice established by University of Michigan Health System 
  •  Practice within the regulations of the Michigan Public Health Code 
  • Collaborate with CCC for addressing all incoming patients calls.
  • Provide support for the red call process in collaboration with the CCC/Call center.
  • Collaborates with the CCC to ensure all patients are communicated and engaged in the plan of care.

Required Qualifications*

NP: 

  • Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
  • Current specialty certification to practice as an Acute Care Nurse Practitioner, Adult Gerontology Acute or Primary Care NP, Adult NP, or Acute NP.
  • Graduate of an accredited Nurse Practitioner program.
  • Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
  • DEA license required, or ability to obtain DEA license within 30 days of start date required

PA:

  • Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
  • Certification by the National Commission on Certification for Physician Assistants (NCCPA).
  • Current license to practice as a Physician Assistant in the State of Michigan.
  • DEA license required, or ability to obtain DEA license within 30 days of start date required

  
NP/PA: 

  • At least 2-5 years’ recent experience in a role as an Advanced Practice Provider caring for Adult cardiology patients and/or Adult Cardiac Surgery patients.
  • Demonstrated strong ability to lead a group and work collaboratively and effectively with a multidisciplinary team.   
  • Recent demonstrated expertise performing H&P’s.
  • Strong ability to coordinate and lead a multidisciplinary team.
  • Demonstrates the ability to think creatively and facilitate innovative solutions to improve access and support growth of a program.
  • Demonstrates a strong ability to work collaboratively with a team to define processes.
  • Demonstrated experience with and knowledge of current medical, surgical and pharmaceutical treatment modalities of the post-cardiothoracic surgical patient or cardiac procedures.   
  • Ability to function independently and partner with team members to create ideal patient outcomes.  
  • Demonstrated commitment to patient and family centered care. 
  •  Current ACLS credential.  
  • Demonstrated high level of critical thinking. 
  • Demonstrated high level of motivation to learn and participate in activities to improve patients care.  
  • Must reflect a professional appearance and demeanor and possess excellent customer service skills.
  • Demonstrated excellent verbal and written communication skills

Desired Qualifications*

  • Demonstrated clinical leadership and experience with coordinating and managing complex and multifaceted projects.  
  • Experience developing a new program or leading innovation
  • Recent demonstrated experience performing H/P specific to the Cardiac Surgery population
  • Experience working as a registered nurse with Adult Cardiac Surgery population.
  • Previous experience in the development of evidence based clinical protocols.  
  • Previous experience in professional presentations and group education.  
  • Previous participation in clinical research.
  • Experience in program development, quality improvement, research and regulatory readiness program planning.  
  • Demonstrated clinical leadership and experience with coordinating and managing complex and multifaceted projects
  • Previous experience in an ambulatory cardiology clinic

Work Schedule

Hours/Week:  40 hours
Day shift - No on-Call /4 days- varied days and hours, could include weekends and/or afternoons in future.
Location: CVC Clinic, could include traveling in the future
Coverage: Time off will be supported in collaboration with the Structural Heart Trans-Catheter Aortic Valve Replacement (TAVR) advanced practice team.
