Manage all aspects of high-risk lab monitoring in Dermatology. The successful applicant will follow a specific protocol for Avitracks and will establish new workflows to monitor over 700 active patients on high-risk medications, utilizing AviTracks and MiChart. Coordinate with Medical Assistants, Patient Services Staff, Residents, and Attending Physicians to provide extremely reliable monitoring of high-risk medications. Provide education to patients regarding high-risk medication monitoring in certain clinics.
Enroll and Monitor Patients
Accurately enroll patients into lab monitoring system in a timely fashion and understand importance of lab monitoring and risks involved with noncompliance. Create lab work orders, as established by protocols or under physician instruction. Manage patient forms: ensure correct delivery to pharmacy/insurance company/home care agency, as appropriate.
Enter lab reports directly into AviTracks from outside labs, initiate phone calls to patients to relay lab results and next steps, complete telephone encounters documenting conversation with patients , and notify physicians of abnormalities.
Make reminder calls to patients, as appropriate. Call laboratories and health care agencies to obtain test results. Follow up on missing/delinquent lab reports and ensure that lab results have been obtained in advance of patient visit. Initiate phone calls and letters to patients for missed labs.
Perform triage. Do complicated Prior Authorizations. Send faxes and image documents into MiChart.
Educate patients on high-risk medication guidelines established by the Department of Dermatology. Able to explain the lab monitoring program and risks of noncompliance to patients, answer patient questions. Confirm that patients understand medication start date, lab frequency, and importance of notifying the department if they go to a different lab than their typical lab. Work with physicians and other staff to revise and develop new patient education materials. If needed, educate patients on routine procedures and treatments, and council patients on wound care, according to established guidelines. Monitor phones, including answering incoming patient questions and making follow-up calls.
Professional Development/Continuing Education and Compliance
Attend continuing education and staff development programs, specifically to maintain current knowledge about dermatologic conditions, medications and treatment modalities, and AviTracks/MiChart. Complete Mandatories. Maintain LPN licensure. Participate in Quality Improvement initiatives. Identify staff educational needs to enhance patient care. Provide for the safety of self and others by adherence to safety guidelines, policies, and practices. Protect patient privacy in accordance with HIPAA regulations.
Direct Patient Care
This position also may be required to assist with providing comprehensive nursing care, including, but not limited to: obtaining health status and patient history, vital signs; dressing, suture/staple removal; ensuring efficient clinic flow, preparing exam rooms for visit/procedure and assist physicians.
Additional duties as assigned.
Graduation from an accredited School of Practical Nursing and current licensure to practice in the State of Michigan.
Excellent written and verbal communication skills, extremely high level of customer service, exceptional organizational skills and very detail oriented.
Very reliable with an excellent attendance record. Team player.
Must exhibit a professional and positive image when interacting with patients, staff, and faculty.
Strong computer skills and ability to adapt to change.
NOTE: Required qualifications must be met by the candidate in order to be interviewed and considered for the position.
Prior experience working with monitoring of laboratory values and medication monitoring, and experience working in an academic medical center.
Experience using MiChart, AviTracks.
