Job Summary

The UM Comprehensive Eating Disorders Program (CEDP) seeks a patient navigator who will be an integral member of the treatment team, serve as a front-line contact for patients preparing to enter our program, and coordinate patient care from the time of initial referral through treatment completion. The patient navigator will conduct initial phone screens of prospective patients, which requires an understanding of eating disorders and associated symptoms as well as the community resources for patients who are unable to be treated in our program. We are seeking an individual who can skillfully provide education to prospective patients about services offered with sensitivity and professionalism. The patient navigator will also manage the scheduling of patient care activities, facilitate patient entry into our program, and coordinate with insurance providers, including obtaining insurance authorizations and completion of utilization/concurrent reviews for patients being treated in the clinic. The patient navigator will deliver pertinent clinical information to multi-disciplinary team during team meetings and follow-through on relevant tasks discussed during the meeting.

Responsibilities*

  • Conduct phone screens, which are used by program nurse and program leaders as an initial assessment of patient symptom severity in order to triage care appropriately. This requires in-depth knowledge related to the symptoms and treatment of eating disorders.
  • Coordinate admissions into the CEDP services with sensitivity and professionalism.
  • Complete insurance authorizations and concurrent/utilization reviews for program participants. This role requires an intimate knowledge of the patients’ condition and requires a clinical perspective when discussing cases with insurance authorizers.
  • Assist with management of incoming calls using the Aspect phone system and provide information about the CEDP (including an overview of the treatment approach and levels of services offered) to potential patients/families.
  • Attend and participate in relevant CEDP multi-disciplinary team meetings.
  • Assist with other clinic coverage and duties as assigned, and interact therapeutically with patients and families while adhering to program treatment model
  • Organize and provide materials to patients in preparation for their evaluations and enrollment into the program.
  • Be available to contact other medical and treatment providers for care coordination.
  • Assist parent(s) to complete forms in visit if needed.
  • Assess patients/families for tangible needs and services and address directly.
  • Secure information for appropriate triage to social workers, Clinic Directors, and CEDP team members.
  • Interface with units such as Billing, Admissions and Security as appropriate for coordination of care.
  • Match patient/family needs with community resources as needed.
  • Collect and record data for staffing and patient care needs.
  • Receive supervision from Clinic Directors, Lead Clinical Social Worker, and Administrative Manager.
  • Serve on Departmental and Institutional Committees as appropriate.
  • Participate in staff development opportunities as appropriate.

Required Qualifications*

  • Bachelor’s degree in Psychology, Counseling, Social Work, Child Development or related field. 
  • 5+ years of related experience
  • Knowledge or strong interest in developmental behavioral pediatrics, psychology, eating disorders and chronic illness. 
  • Excellent interpersonal and customer service skills.  Ability to convey warmth and compassion over the phone. Comfort in working with adolescents, young adults, and their families.
  • Accurate, thorough and timely completion of tasks with demonstrated ability to prioritize.
  • Demonstrated ability to work as part of an interdisciplinary team with other health professionals. 
  • Strong computer and telephone skills, excellent verbal and written communication skills.

 

Desired Qualifications*

  • Experience working with patients with mental health disorders
  • Familiarity with mental health insurance benefits, including experience completing insurance authorization and/or concurrent/utilization reviews
  • Working knowledge of MiChart or Epic EMR
  • Knowledge of medical and mental health terminology
  • Experience working with adolescents, young adults, and families
  • Able to manage multiple responsibilities in a busy clinical setting, possessing strong problem solving skills and attention to detail

Work Schedule

Monday - Friday, 7:30 a.m. - 4:00 p.m.
