Job Summary

This position is responsible for providing occupational therapy services to patients in their homes as ordered by a physician.  The occupational therapist acts as team member working as a supplemental discipline in a shared nursing/therapy case.  The occupational therapist has responsibility for a patient caseload and for communicating with other members of the interdisciplinary team to promote continuing coordination of the patients services. 

This full time position requires travel throughout southeast Michigan


  • Completes comprehensive occupational therapy assessments to identify the occupational therapy needs of each assigned patient.
  • Provides ordered occupational therapy for all nursing/therapy shared cases and keeps case manager aware of therapy progress and status on a regular basis.
  • Assesses patients level of functioning and develops and updates a plan of care for patients in their home environment based on the initial and ongoing assessment of patient needs in compliance with all regulations, policies and procedures within regulated time frames. 
  • Achieves high quality patient outcomes as well as accomplishment of the plan of care.
  • Responsible for accurate and timely completion of OASIS documentation
  • Uses point of care documentation efficiently and effectively when applicable.
  • Communicates and interacts with patients/caregivers and families in a positive and effective manner.
  • Teaches and/or supervises family members and other caregivers in the occupational therapy needs of the patient.  Encourages and assists patients and caregivers to achieve their maximum degree of independence.
  • Complies with applicable continuing education requirements.

Required Qualifications*

  • Bachelor's Degree from an accredited program and current certification/licensure as an Occupational Therapist in the State of Michigan
  • Two (2) years of recent and relevant occupational therapy experience 
  • Possession of comprehensive skills including OT evaluation and treatment
  • Ability to work autonomously in a non-structured environment
  • Strong organizational skills
  • Effective verbal and written communication skills
  • Proficient computer skills and ability to perform computer documentation
  • Able to travel from patient to patient throughout the geographic service area assigned

Desired Qualifications*

  • Recent, within the last 5 years,home health care experience
  • Direct care of  pediatric population
  • Lymphedema and/or LSVT certification
