Adult Kidney Transplant Social Worker: Patient evaluation, education and counseling services to kidney transplant candidates and recipients. Provide services to patients and families, liaison and referral to community agencies, participation in interdisciplinary team meetings and related educational and research activities for transplant candidates and their families. Includes both inpatient and outpatient work, with both pre- and post-transplant candidates/recipients.
Master's degree in Social Work from a school accredited by the Council of Social Work Education is necessary. Must have current clinical license to practice social work in the State of Michigan. Clinical practice concentration and field practicum or recent work experience in a hospital required. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification
6 or more months of recent medical hospital social work experience, casework and group work education and experience. Social Work experience with adult population in medical health care setting. Experience working within interdisciplinary teams, providing services to patients and families. Knowledge of community resources. Experience with program planning and development; support and education groups. Also skills and experience in the following: ability to establish therapeutic relationships quickly, and to relate well with diverse client populations. Ability to manage large caseloads with multiple competing demands. Crisis intervention and grief counseling. Familiarity with ESRD programs, regulations, treatments, as well as appropriate resources. In-depth psychosocial assessment of potential transplant recipients. Knowledge of psychosocial issues associated with solid organ transplant. Substance abuse assessment, referral, liaison with treatment agency; development of treatment plan for these patient populations
