A total of one(1), 1.0 FTE, Advanced Practice Professional (either NP or PA) will be hired to fill this job opening.


This Advanced Practice Provider will provide health care and specialized health services to patients and families enrolled in the Cardio-oncology/General Cardiology programs.    

  • Under physician supervision, the Advanced Practice Provider will coordinate care of clinic patient’s, perform consultations, pre-op evaluation and history, and follow up with the patients, including but not limited to placing orders and following up on patients testing.
  • Participate in clinical trials now and in future as necessary.  
  • Provide Direct Advanced Practice support for the Cardio-Oncologist and General Cardiologist.
  • Will collaborate with other care providers/consultants to coordinate and communicate clinical care.
  • Provide leadership and demonstrate strong skills of coordinating a multidisciplinary approach to support the patients. 
  • Potential rounding on inpatient setting for new consults and collaborate with inpatient clinical team/Ambulatory CCC to facilitate transitions of care.
  • Facilitate transitions of care coordinating bridge clinic for general cardiology/cardiology Onc patients to be seen within 7-14 days’ post hospital discharge.  
  • Facilitate a seamless patient centered care transition from time of discharge, coordinating with appropriate physician until patient is seen for their first outpatient Cardiology appointment.
  • Will also provide clinical expertise and direction to support patient medication management
  • Participate in monthly meetings, case presentation, related to patient population.
  • Maintain accurate documentation regarding patient status, evaluations, and treatments to physicians, the RN/Clinical Team; streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external UMHS team members and other ambulatory care providers
  • Participate in quality assurance activities, process improvement projects (LEAN), and facilitate growth.
  • Perform other related duties as required to assure patient safety and satisfaction
  • Participate in the development and presentation of educational experiences for patients, families and health care professionals.
  • Utilize Service Excellence and Patient Family Centered Care in daily practice
  • Abide by the standards of care and standards of practice established by University of Michigan Health System 
  •  Practice within the regulations of the Michigan Public Health Code 
  • Collaborate with CCC for addressing all incoming patients calls.
  • Provide support for the red call process in collaboration with the CCC/Call center.
  • Collaborates with the CCC to ensure all patients are communicated to and engaged in the plan of care.

Required Qualifications*


  • Current license to practice as a Physician Assistant in the State of Michigan
  • Satisfactory completion of a Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA), or one of its predecessor agencies, CAHEA or CAAHEP  
  • DEA license required, or ability to obtain DEA license within 30 days of start date required


  • Current license to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
  • Certification as an Acute Care Nurse Practitioner, Adult Gerontology Acute or Primary Care NP, Adult NP, or Acute NP recognized by the National Council of State Boards of Nursing (NCSBN).   
  • DEA license required, or ability to obtain DEA license within 30 days of start date required


  • At least 2-5 years’ recent experience in a role as an Advanced Practice Provider caring for Adult cardiology patients.
  • Any experience caring for Adult Oncology patients.
  • Demonstrate strong ability to lead and work collaboratively and effectively with a multidisciplinary team.   
  • Demonstrates the ability to think creatively and facilitate innovative solutions to improve access and support growth of a program.
  • Demonstrated experience with and knowledge of current medical, surgical and pharmaceutical treatment modalities for Cardiac Patients.  
  • Ability to function independently and partner with team members to create ideal patient outcomes.  
  • Demonstrated commitment to patient and family centered care. 
  •  Current ACLS credential.  
  • Demonstrated high level of critical thinking. 
  • Demonstrated high level of motivation to learn and participate in activities to improve patients care.  
  • Must reflect a professional appearance and demeanor and possess excellent customer service skills.
  • Demonstrated excellent verbal and written communication skills

  • Previous participation in clinical research.
  • Experience in program development, quality improvement, research and regulatory readiness program planning.  
  • Demonstrated clinical leadership and experience with coordinating and managing complex and multifaceted projects

Work Schedule

Hours/Week:  40 hours=Four- 10 hour shifts- 80%clinic, 20% prep 
Day shift – Monday, Tuesday, Wednesday, and Friday.  Thursday-Day off.  
Future Schedule could change-days and hours varied, could include weekends and/or afternoons.
Location: Primary CVC Clinic and Northville Health Center, could include traveling to other off site clinics in the future.
