Basic Function and Responsibility
To work as part of a team to assist in the development, implementation and monitoring of clinical pharmacy services in Pediatric Cardiology for the Pharmacy Department.
To work collaboratively with the current clinical pharmacy specialist to develop and provide services in the pediatric cardiothoracic intensive care unit, on the general care floor, as well as to the pediatric heart transplant and pulmonary hypertension teams.
Daily Clinical Pharmacy Specialist Responsibilities include:
Daily patient care rounds
Pediatric cardiothoracic intensive care unit (2 teams)
General care floor (1 surgical and 1 medical team)
Each clinical pharmacy specialist would cover one ICU team and 1 general care floor team
Transitions of care activities
Heart transplant team
Monitor immunosuppressant therapies and provide recommendations
Answer drug information questions
Provide transplant education to transplant patients and families and document in the electronic medical record
Attend mandatory transplant evaluation meeting weekly
Pediatric pulmonary hypertension team
Provide recommendations for prostacyclin therapy
Answer drug information questions
Provide education to physician and nursing groups
Preceptor – Pharmacy students, PGY1 resident, PGY2 resident in pediatrics
To ensure and promote the safe, rational, efficacious and cost-effective use of drug therapy.
To work as part of the team to develop guidelines and policies and to assist in the clinical training of medical staff, pharmacy staff, residents and students.
Characteristic Duties and Responsibilities
Assist in the development, implementation and monitoring of clinical pharmacy practices consistent with the Health Systems needs and objectives.
Provide pharmaceutical care by: active participation in multidisciplinary rounds on an ICU team and general care team; designing, implementing and monitoring patient-specific pharmacotherapy, including pharmacokinetic support, nutrition support and anticoagulation management; and documenting pharmaceutical care activities.
Assist in the development of staff educational and training programs within and relating to the Pediatric Cardiology service line.
Actively participate in the training of staff, residents, and students from the College of Pharmacy.
Lead drug use projects, cost reduction/management initiatives, and quality improvement activities consistent with the Health Systems needs and objectives.
Individual or collaborative scholarly/research activity is encouraged.
Provide drug information and education to patients and caregivers.
Participate in UMHS committee(s) related to the care of pediatric cardiology patients.
Local and National Professional Organization Involvement is encouraged.

- A Doctor of Pharmacy degree or an equivalent combination of education and experience is necessary.
- A certificate of completion from a specialty residency in pediatrics, fellowship, or an equivalent combination of education, training, and experience is necessary.
- Experience in clinical pharmacy practice is necessary.
- Licensure as a Pharmacist by the State of Michigan is necessary.
- Possession of a state controlled substance license is necessary.
Ability to perform a variety of manual dexterity movements, including keyboarding and IV preparation, actively participating in codes. Fluency in reading, writing and speaking English.
- Completion of a PGY2 pediatric pharmacy residency or an equivalent combination of education, training, and experience is preferred.
- Board of Pharmacy Specialties certification is preferred.
- Background in intensive care and/or nutrition support is preferred.
This position may be underfilled at a lower classification depending on the qualifications of the selected candidate.
- General supervision is received from the assistant director and /or team leader or other designated official.
- Functional supervision is exercised over pharmacy residents, pharmacy technicians, students, and other staff performing delegated pharmacy services.
