Responsibilities*

Responsibilities include but are not limited to:
-Completes comprehensive SLP assessments during home visits to identify the speech, language and swallowing needs of each assigned pediatric or adult patient.
-When SLP is the case manager: secures and maintains complete and accurate physician orders within the regulated time frames. Coordinates the plan of care with other staff and is responsible to document this coordination for inclusion in the medical record.
-Provides ordered speech therapy for all nursing/therapy shared cases and keeps case manager aware of therapy progress and status on regular basis.
-Develops and updates a Plan of Care based on the initial and ongoing assessment of patient needs meeting all regulations and time frames.
-Performs and documents therapy observations, procedures, and interventions in the home according to the established Plan of Care and in compliance with all policies and state and federal regulations.
-Participates in department development by sharing observations about individual and aggregate health care needs, program planning needs, etc., to administrative staff of the department.
-Provides care based on the standard speech language pathology processes, medical orders, mission and protocols and Patients Bill of Rights.
-Teaches and/or supervises family members and other caregivers in speech therapy home programs and techniques to meet the needs of the patient. Encourages and assists patients and caregivers to achieve their maximum degree of independence.
-Communicates and interacts with patient/caregivers and families in a positive and effective manner.
-Processes all documentation according to department schedules to ensure inclusion in the medical record within regulated time frames.
-Coordinates the plan of care with other staff: nursing, occupational therapy, physical therapy, home health aides, dietitian and medical social work staff of the department for all primary speech language pathology cases, and is responsible to document this coordination for inclusion in the medical record.
-Evaluates and supervises speech therapy services
-Participates in program development and research activities as directed

Required Qualifications*

-Master's degree from an accredited program in the field of Speech & Language Pathology or Communication Science
-Current membership in American Speech and Hearing Association
-Certificate of clinical competence (CCC)
-One year of SLP experience in an acute hospital, rehabilitation center, long-term care facility or home care
-Computer proficiency
-Current valid, unrestricted Michigan driver's license
-Ability to work autonomously in a non-structured environment
-Extensive experience in speech and language testing
-Excellent verbal and written skills
-Under Michigan Law, a criminal history check including fingerprinting is required as a condition of employment or transfer for this position.

Required Qualifications for Senior Level Positions:

Two or more years of SLP experience in an acute hospital, rehabilitation center, long-term care facility or home care.

Physical Requirements/Demands:

The employee will be able to perform the assigned duties and responsibilities with or without reasonable accommodations.

The position requires extensive standing/walking as well as lifting, positioning, pushing and/or transferring of patients. This position requires knowledge of the use of laptop computers, and cell phones for documentation and communication.   These duties require fine motor coordination/dexterity, the ability to grasp, vision with or without corrective aids adequate for fine motor tasks, client assessment, and driving.  The job requires the physical demands of occasional lifting greater than 50 pounds, reaching, pushing, walking, going up and down stairs, driving, etc., with or without the use of assistive/adaptive devices.  Employee drives personal or University owned vehicle to various work site locations on a regular basis including in-town and highway driving in good and inclement weather.
