Job Summary

Coordinates all aspects of the evaluation/listing process for deceased donor and/or living donor transplantation for Chronic Kidney Disease patients.  Coordinates organ procurement with the Gift of Life Michigan (GOLM) Organ Procurement Organization through shared on-call responsibility.  Acts as liaison between the University of Michigan and all referring dialysis centers and referring MDs.  Ensures continuity of care through effective communication and collaboration with the multi-disciplinary team in all areas of the transplant department.  Uses an understanding of the principles of growth and development to assess each patient’s age specific needs and provide age specific treatment.  

Responsibilities*

Uses an understanding of the principles of growth and development to assess each patient’s age specific needs and provide age specific treatment.

Job-Specific Requirements :
1. Identifies and evaluates the health status of patients in need of kidney and/or pancreas transplantation.
2. Coordinates pre-transplant work-up and evaluation for both deceased donor and living donor transplants. Entering all nursing documentation, including test results in OTIS.
3. Evaluates all transplant referrals as candidates for deceased/living donor transplantation following established procedures and documents appropriately.
4. Takes on-call responsibility (24 hours/day) including a minimum of four days per month, to coordinate acceptance and procurement of organs with the transplant physician through the GOLM. Shares this responsibility with Statline. 
5. Completes nursing assessment on all new transplant candidates. Maintains medical records for patients in active caseload following the established protocol.
6. Communicates assessment data and findings to other members of the transplant team.
7. Coordinates all aspects of pre-transplant education program as assigned.
8. Collaborates with multi-disciplinary team during transplant evaluation meetings, shares appropriate patient information and in-put to help make the best decision in listing/delisting of patients. 
9. Provides ongoing in-services and continuing education programs on transplantation to other health care professionals, team members, patients and members of the community, upon request.
10. Ensures that required histocompatibility testing, antibody screening, and other pertinent specimens or tests are performed for all potential transplant patients.
11. Maintains active evaluation and waiting list regularly.
12. Participates in the development and implementation of policies and procedures of the transplant department.
13. Acts as liaison between the University of Michigan, UNOS and GOLM. 
14. Completes necessary documentation for tissue typing, candidate listing, donor/recipient cross-match, etc through GOLM and UNOS as appropriate.
15. Participates in and contributes quality assurance and improvement programs of the Transplant Department.
16. Completes all relevant UNOS forms at time of listing of transplant candidate or living donor transplantation through TIEDI.
17. Meets with assigned physician weekly to review complex cases as well as cases that are not progressing to listing.
18. Appropriately prepares and presents cases to the Kidney Evaluation Team and completes follow-up actions as indicated.
19. Completes review of entire caseload at least annually, initiating appropriate contact with the patient and documenting action(s) taken in OTIS.
20. Acts as a team player by assisting other transplant coordinators and administrative team as needed.
21. Attends Outreach clinic 1-2 per month

Salary & Nursing Framework Level:
This UMPNC RN posting is posted as Nursing Framework REG NURSE - LEVEL C (B INC).
Actual Nursing Framework LEVEL and salary will be determined at time of hire.
Nursing Framework levels range from Level A to Level E.

Required Qualifications*

  • Current licensure to practice nursing in the State of Michigan.
  • Ability to provide leadership, prioritize patient needs and manage multiple tasks simultaneously.
  • Excellent written and verbal communication skills as demonstrated by resume and performance reviews.
  • Demonstrated collaborative behavior in working with peers and the multidisciplinary team.
  • Demonstrated problem solving skills and independent clinical decision making as evidenced by current documentation practice.
  • Demonstrates excellent attendance and punctuality as evidenced by performance evaluation.
  • Minimum of 3 years’ experience with medical/surgical patient population.
  • Minimal of 1 year of recent experience with transplant adult patient population
  • Experience in caring for patients post operatively within the last 3 years.

Desired Qualifications*

  • Minimum of 2 years in an Ambulatory Clinic.
  • BSN
  • Ability to effectively navigate computer systems including competence with Careweb, MiChart, OTIS, UNOS, Microsoft Word/Excel and Outlook.
  • Experience with reviewing and interpreting HLA/Tissue Typing data
  • LEAN training knowledge.
  • Certified Clinical Transplant Coordinator (CCTC) certification, preferred, and required within 2 years of employment (effective 7/1/15). 
  • Experience with immunosuppression management.
  • Minimum 1 year experience with ambulatory care.

Hours per week:  40
Shift/Days/Hours: Day Shift; Monday through Friday; specific hours and days to be determined
Location: Transplant Center- Taubman Center; Pre-Renal Transplant Coordinator


Note: All new employees will be expected to float to a designated unit(s) in times of low census.
