This half-time (0.50 FTE) position is for a Clinical Social Worker at Burlington Physical Medicine and Rehabilitation Clinic. The patient population is comprised of adult and geriatric patients with medical diagnoses including cerebral palsy, spinal cord injury, stroke, cancer, and cognitive impairment. Social work referrals will be initiated by clinic providers for issues related to patient education, crisis intervention, safety planning, care/case management, and coordination of care.
The social worker will provide psychosocial assessments, treatment and case management services to patients and their families, face-to-face and telephone contacts with patients and/or their families, and brief therapeutic interventions using evidence-based treatment modalities
A Master's degree (MSW) in social work from a school accredited by the Council of Social Work Education is necessary. Candidates must have a current clinical license (LMSW) to practice social work in the State of Michigan, with clinical certification. LLMSWs will not be considered for this position.
This position requires efficient, accurate assessment of patient needs, excellent verbal and written communication skills, ability to quickly develop therapeutic relationships with a diverse patient population, excellent organizational skills, maintaining accurate and timely clinical records, exercising independent decision-making and critical thinking skills, participation in team meetings, and the ability to effectively prioritize multiple competing tasks. It is essential to be able to work independently in a variety of demanding situations, utilizing critical decision making skills. The successful candidate must be flexible, adaptive, be able to work effectively with a multidisciplinary team, and able to access community resources. Experience providing both medical and clinical social work interventions is desirable
This is a 20-hour (0.50 FTE) position, with the hours spread over three days. Specific days and hours will be determined with clinic leadership; some flexibility may be required. This position will report to the Social Work Department with matrix reporting to the Burlington Physical Medicine and Rehab Clinic Manager and Medical Director.
This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification
