Job Summary

This position holds two primary responsibilities:  Care Management Pediatric Social Work supervisor and C&W Advanced Care Management Team Program manager.

Responsibilities*

Clinical Care Management Social Work Supervisor (0.5 FTE): Responsible for the daily operations of the pediatric clinical social work area, including clinical supervision, case consultation, resource information maintenance, and review of chart documentation.  Provide individual and group supervision and case consultation according to Department guidelines. Participate in hiring, onboarding, and orienting of new staff. Complete bi-annual supervisory chart audits, assist in disciplinary actions in conjunction with the Care Management Social Work Manager, and compilation of and follow up from documentation/disciplinary action plans.  Plan and conduct training to staff as needed and participate in performance reviews.

Clinical Care Management Social Work Supervisor Duties and Responsibilities:

  • Provide clinical case consultation to staff, as well as clinical supervision, both individual and group.
  • Provide specialty consultation to other health care staff.
  • Complete quarterly supervisory chart audits and ensure any corrections and stay current with billing and insurance requirements for charting.
  • Assure adequate staffing by managing scheduled absences.
  • Participate in and lead quality improvement projects related to clinical documentation and outcomes.
  • Facilitates workgroup, Employee Engagement Activities, and Group Supervisions, developing agendas and meeting minutes as appropriate.
  • Participate as a member of an inter- and intra-disciplinary team, including problem solving, program planning, clinical care planning and quality improvement.
  • Participate in Departmental Committees as well as C&W Organizational Committees and Activities relevant to Care Management.

C&W ACMT Program Manager (0.5 FTE): This position is responsible for managing the 5-8 most psychosocially, behaviorally and systematically complex cases hospitalized in C&W. This position functions to remove systematic barriers to care, expedite discharge of complex cases, and reduce long length of stay.  This role requires significant pediatric social work experience, strong assessment skills, the ability to establish therapeutic relationships quickly, and to relate to a diverse population.  It is essential that the candidate be able to manage a complex caseload with multiple competing demands.  The ability to work independently in a variety of stressful and demanding situations is essential; critical decision making, brief interventions and multi-tasking are only several of the advanced skills required for success.  Must be able to work with a multidisciplinary team and access community resources in order to facilitate transitions and discharge planning.  Experience providing crisis intervention and grief counseling is essential, along with brief interventions to facilitate patient and family participation in appropriate health care decision making.

 

C&W Advanced Care Management Program Manager Duties and Responsibilities:

  • On the basis of preliminary risk screening, assess patient and family’s psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness and ability to cope.
  • Assume responsibility for difficult and complex cases based on experience or area of expertise as needed to facilitate successful transition plan.
  • Appropriately documents in appropriate system, those patients identified as high risk for complex case management and enters all interventions.
  • Strong collaboration with Care Management staff for early notification of complex patients.
  • Flagging patients using daily reports generated to assist with the identification of at risk patients.
  • Participated in discharge planning activities for complex patients, in order to ensure a timely discharge and to provide appropriate linkage with post discharge care providers.
  • Aligns with families exhibiting complex family dynamics that impact directly on patient care and discharge.
  • Communicates with the healthcare team regarding the discharge planning status of all patients referred.
  • Educates patient, family, care team, post-acute options and addresses issues of choice.
  • Identifies reports and resolves actual and potential delays in service and inappropriate use of resources.
  • Participates in the development of a plan of care with the multi-disciplinary healthcare team, patient, family and/or caregiver.
  • Identifies and refers potential quality concerns to the Advanced Care Management Planning Committee who would then refer to the Executive Leadership Committee.  Ad hoc members for consultation would include quality, Risk Management, and Legal Affairs.

Required Qualifications*

Intermediate Required Qualifications:

  • A Master’s Degree in Social Work from a School Accredited by the Council of Social Work Education (CSWE) is necessary.
  • 3 or more years of professional social work experience is preferred.
  • Current Licensure (LMSW) to practice social work in the State of Michigan is required.
  • Demonstrated excellent written and verbal communication skills.
  • Demonstrated experience in a supervisory role or demonstration of leadership skills preferred.
  • Prior MSW Clinical Experience in an inpatient hospital setting is preferred.
  • Experience with leading department or organizational projects

 Senior Required Qualifications:

  • A Master’s Degree in Social Work from a School Accredited by the Council of Social Work Education (CSWE) is necessary.
  • 5 or more years of professional social work experience is preferred.
  • Current Licensure (LMSW) to practice social work in the State of Michigan is required.
  • Demonstrated excellent written and verbal communication skills.
  • Demonstrated experience in a supervisory role or demonstration of leadership skills.
  • Prior MSW Clinical Experience in an inpatient hospital setting is required.

 

Desired Qualifications*

The ideal candidate will have significant pediatric, mother/baby, and ICU experience. Strong assessment skills and the ability to establish strong therapeutic relationships quickly is desired. The ideal candidate will relate well with a diverse population, having a strong sense of cultural humility and attention to social justice issues.

Additional Information

Supervision Received:

Clinical and Administrative Supervision is received from Care Management Associate Director.

Work Schedule:

Standard work hours:  8:00 am – 4:30 pm, Monday through Friday.  Work hours may vary beyond the standard work hours due to clinical patient needs of complex patients and staff supervisory needs on some occasions.
