This is a multi-unit RN position which involves staffing assignments with Michigan Visiting Nurses, Michigan Visiting Care and Home Med. The home unit will be Michigan Visiting Nurses and PTO will be determined from the home unit. The RN can be scheduled in any of the 3 units, based on the staffing needs.
Michigan Visiting Nurses-Home Unit
Provide skilled home care nursing services during intermittent visits to multiple patients on a daily basis.
Assess physical, psychosocial and environmental needs of patients and families in accordance with agency, state and federal regulations.
Coordinate care for assigned caseload of patients and families as a member of a multi-disciplinary home health team.
Document appropriately via Point of Care computer documentation.
Michigan Visiting Care
Provides skilled nursing services to patients in their homes and is responsible for assessing nursing needs, including the physical, psychosocial and environmental needs of patients and their families. May act as a primary nurse to one or more patients, encompassing the responsibilities of updating care plans, medication profiles and maintaining organization of the medical chart in the home. The home health RN safely completes procedures and treatments as necessary.
As a member of a multi-disciplinary home health staff, the RN ensures the provision of safe delivery of nursing care following all department policies and state and federal regulations
This position is responsible for the provision of nursing care, assessment, evaluation, training and monitoring to an assigned patient population. Primary responsibilities are to provide initial and ongoing infusion nursing care, including the administration of intravenous specialty medications to patients in the home complying with applicable laws, regulations and policies. Triages problems as they arise with home infusion and participates in on-call responsibilities as assigned.

Current licensure to practice nursing in the State of Michigan
Two (2) or more years of acute care RN experience in adult medical-surgical and/or home care RN expeirence in the last five (5) years
Direct experience with all types of vascular access devices, wound care, and oxygen therapies
In-depth experience in patient education (i.e. self-care, disease management state)
Ability to travel from point A to point B throughout the workday as assigned
