Job Summary

To perform nutrition assessments and develop nutrition care plans using the Academy of Nutrition Care Process for in-center and home therapy patients. To provide recommendations for nutrition intervention, participate in interdisciplinary care plan meetings and conduct nutrition education to patients. Participate in Quality Assessment and Performance Improvement (QAPI). Display a high level of customer service, attentiveness and consideration to patient care while providing evidence-based medical nutrition therapy.

General supervision is received from Clinical Nursing Director (CND) and/or Clinical Nursing Supervisor.

Responsibilities*

A.  Patient Treatment and Documentation

  • Assesses patient’s knowledge of diet and kidney disease and provides education appropriate to patient’s learning needs regarding the relationship of diet and kidney disease and the impact on the treatment process.
  • Provides patient specific detailed education to patient/family/caregiver regarding nutritional status, adequacy measures and results, fluid status and all nutrition related lab parameters pertaining to their disease process.
  • Completes nutritional assessment and participates in the interdisciplinary comprehensive assessments.
  • Participates with the interdisciplinary team in the development of a written, individualized comprehensive plan of care that specifies the service necessary to address the patient’s needs, in accordance with established Federal and State licensure guidelines.
  • Calculates diet prescription according to standard of practice.
  • Completes progress notes on all chronic dialysis patients. Maintains complete and accurate documentation of all appropriate information in the electronic medical record according to unit policies.
  • Provides patient with ongoing nutrition assessment and outcome-oriented nutrition counseling necessary to assist patient in achieving and sustaining and effective nutritional status.
  • Identifies malnourished patients as well as patients at risk for malnutrition and works collaboratively with interdisciplinary team to identify appropriate interventions, resources or solutions. Review’s each patient’s albumin level and body weight monthly. Implements plan of care with involvement of patient/family/caregiver.
  • Monitors adherence and responds to nutrition therapy, adjusts therapy as needed to achieve and sustain an effective nutritional status, documents why the patient was unable to achieve the goals and implements the plan of care to address issues impacting the patient’s ability to achieve the goals.
  • Collaborates with the interdisciplinary team on Urea Modeling recommendations to achieve the physician’s prescription of Kt/V.
  • Collaborates with the interdisciplinary team on management and recommendations for mineral metabolism and renal bone disease, anemia and iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation status and sodium/volume control.
  • Reports on nutrition QAPI results and participates in the interdisciplinary QAPI program.
  • Reviews nutrition related laboratory results.
  1. Counsels patient/family/caregiver and formulates appropriate action.
  2. Appropriately recommends treatment changes to the interdisciplinary team.
  3. Appropriately communicates with physician and facility staff regarding lab results.
  4. Communicates lab results and recommended treatment changes to providers of patient care in nursing facilities including hospitals and short and long term care facilities.
  • Participates in staff meetings, care plan meetings and other meetings as applicable.
  • Maintains and improves knowledge and skills for a competent and innovative practice.
  • Maintains diabetic registration and continuing education hours as specified by American Dietetic Association.
  • Collaborates with interdisciplinary team, ensuring that all work areas are safe and clean.
  • Other nutritional management duties as assigned.

B.  Professionalism and confidentiality

  • Personal business will not be conducted in patient care areas.
  • Maintain patient confidentiality.
  • Maintain confidentiality around performance and personnel issues that arise for staff.

C.  Other duties as assigned.

Required Qualifications*

  • Bachelor’s degree granted by a U.S. regionally accredited University or College or foreign equivalent, and course work through an Accreditation Counsel for Education in Nutrition and Dietetics (ACNED) accredited Didactic Program in Dietetics (DPD) for Coordinated Program in Dietetics (CP).
  • Completion of 400 hours of supervised practice through ACEND Accredited Dietetic Internship, Coordinated Program in Dietetics or an Individualized Supervised Practice Pathway (ISPP) offered through an ACEND accredited program.
  • Registered Dietician Nutritionist (RDN) with an active registration by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics.

Desired Qualifications*

  • Certified Specialist in Renal Nutrition (CSR).
  • Master’s Degree in nutrition-related field.
  • Minimum of one-year experience in renal nutrition caring for end-stage renal disease patients.
