Responsibilities*

The Inherited Cardiomyopathy and Arrhythmia Program is a vibrant and growing clinical program that manages patients with both genetic cardiomyopathies (such as hypertrophic cardiomyopathy) and genetic arrhythmia syndromes (such as long QT syndrome).  This Advanced Practice Provider will provide health care and specialized health services to patients and families in both the ambulatory care Inherited Cardiomyopathy and Arrhythmia Program as well as in the ambulatory care General Cardiology clinic.  Although the proportion of patients seen in each setting will depend on current clinic volume, it is estimated that the majority of visits will be with the Inherited Cardiomyopathy and Arrhythmia Program, with the remaining clinical effort being in the General Cardiology Bridge Clinic, a dedicated outpatient clinic that facilitates transition of cardiology patients from the inpatient to outpatient setting.     

  • Under physician supervision, the Advanced Practice Provider will coordinate care of clinic patients, perform health assessments including patient history/physical examination, diagnostic tests, referrals, assign appropriate interventions, place orders, follow up with the patients, and monitor outcomes of provided clinical care and additional duties with the scope of licensure.
  • Education focused towards age-appropriate disease management and counseling.
  • Evaluate and interpret reports from diagnostic lab tests, radiographic, cardiovascular procedures, other clinical studies.
  • Follow up will include but is not limited to patients with established diagnoses, or family members of clinically affected patients who are being evaluated for presence of the particular cardiomyopathy or arrhythmia syndrome in their family.
  • Facilitate transitions of care coordinating bridge clinic for general cardiology/HCM patients to be seen within 7-14 days’ post hospital discharge.  
  • Facilitate a seamless patient centered care transition from time of discharge, coordinating with appropriate physician until patient is seen for their first outpatient Cardiology appointment.
  • The Advanced Practice Provider will maintain accurate documentation regarding patient status, evaluations, and treatments to physicians, the RN/Clinical Team; streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external UMHS team members and other ambulatory care providers
  • Will also provide clinical expertise and direction to support patient medication management.
  • Practice will be in partnership with HCM/Cardiology attending, faculty and fellows.
  • Collaboration with the inpatient, OR, cath lab and outpatient teams to develop safe, effective and efficient care coordination for this patient population.
  • Will collaborate with other care providers/consultants to coordinate and communicate clinical care.
  • Coordination of care between clinical care coordinators, physicians and other members of the health care continuum. 
  • Participate in quality assurance activities, process improvement projects (LEAN), and facilitate growth.
  • Participate in the development of policies, procedures and protocols including standards of care.
  • Participate in research projects. 
  • Participate in monthly meetings, case presentations, and additional meetings related to patient population.
  • Perform other related duties as required to assure patient safety and satisfaction.
  • Participate in the development and presentation of educational experiences for patients, families and health care professionals including workshops, lectures, and in-service programs.
  • Utilize Service Excellence and Patient Family Centered Care in daily practice
  • Abide by the standards of care and standards of practice established by University of Michigan Health System 
  • Practice within the regulations of the Michigan Public Health Code 
  • Provide leadership and demonstrate strong skills of coordinating a multidisciplinary approach to support the patients. 
  • Serve as a preceptor for graduate nursing and nurse practitioner students.

Required Qualifications*

Nurse Practitioner Requirements: 

  • Current licensure to practice as a Registered Nurse in the State of Michigan
  • Current specialty certification to practice as a Nurse Practitioner in the State of Michigan 
  • Certification as an Acute Care Adult Gerontology Nurse Practitioner or Primary Care Adult Gerontology Nurse Practitioner or Family Nurse Practitioner
  • Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN) 

Physician Assistant Requirements:

  • Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
  • Certification by the National Commission on Certification for Physician Assistants (NCCPA).
  • Current license to practice as a Physician Assistant in the State of Michigan.
  • DEA required, or ability to obtain a DEA within 30 days of start date required.

Desired Qualifications*

  • One or more years of recent Nursing or Advance Practice experience with cardiomyopathies, heart failure, arrhythmias, and/or cardiac surgery.  
  • At least two years working full time in an advance practice role, preferably in Cardiology
  • Flexibility to adjust future role as current demand/practice/and program grows.
  • Recent experience providing care to patients with structural heart disease. 
  • No specific training in genetic cardiac conditions is required.  However, the individual must be strongly motivated to develop clinical proficiency in inherited cardiac conditions through training in the clinic.
  • Demonstrated knowledge of adult cardiac physiology.
  • Interprets telemetry and basic ECG.
  • ACLS certified with-in 6 months of hire
  • Demonstrated ability to anticipate the clinical care needs of complex patients.
  • Must demonstrate a high level of independent problem solving and decision making abilities. 
  • Demonstrated expert analytical skills and ability to prioritize assignments and manage busy work environment
  • Ability to demonstrate good computer skills including computer documentation and order entry.
  • Demonstrated clinical leadership and experience with coordinating and managing complex, patients and providing safe hand offs to the next provider. 
  • Demonstrated expertise in facilitating the educational growth of others through previous clinical instructional roles

Work Schedule

Hours/Week: 40 hours/week
Shift/Days: Flexible hours and days potential evening and weekends. Flexible start and stop times. 10 hour shifts/flexible day off.
Location: Cardiovascular Center Clinics or other off-site locations as necessary
Work: 80% patient care, 20% administrative time.
