Job Summary

This position is responsible for providing biopsychosocial assessment and case management to home care patients and their families to promote optimal health, maximum functioning and quality of life as ordered by the physician.

Evaluates psychosocial needs and provides social work interventions with a high degree of autonomy, exercising independent judgment and initiative in addressing complex situations. Provides services that are patient-centered, understandable, respectful, and compatible with cultural health beliefs and practices, and in the preferred language.

Utilizes social work techniques and develops a plan of care which is signed by the patient’s physician. Coordinates care with other home health staff and engages the patient, patient representative (if any) and/or family members in the patient’s plan of care to ensure understanding of the impact the disease or illness on the patient’s mental and emotional health, and to provide therapeutic interventions to support necessary lifestyle and behavior changes. Responds to crisis situations and engages with patients and families to access the appropriate level of care across the continuum. Develops strategies to ensure cost effective, quality care outcomes to patients with complex care needs.

Responsibilities*

Provides services that are ordered by the physician as indicated in the plan of care.

- Conducts phone consultations with patients, patient representative (if any) and families to identify needs and to coordinate community resources.

- Visits patients in their homes to assess the relationship between the requirements of the patient’s medical, nursing, and therapy to the patient’s home situation, financial resources and the community resources available to the patient, identifying any barriers to the plan of care

- Develops, evaluates and revises the plan of care based on the initial and ongoing patient assessment, and in partnership with the patient representative (if any) and care givers.

- Assists patients with the social, cultural and psychological factors of their illness and health care regime in the home, including the patient’s representative (if any) and family as appropriate.

- Assists in coordinating patient’s social work plan with the physician, case manager and other agency staff.

- Consults with and serves as a resource person to other health care disciplines regarding social work services administered in the home and projected treatment goals.

- Maintains access to up-to-date community resources.

- Performs and documents social work observations, including patient and family response and compliance with care, and procedures and interventions in the home according to the established Plan of Care according to all agency policies and state and federal regulations.

- Completes documentation according to established department timelines to ensure inclusion in the medical record within regulated time frames.

- Provides services based on the standard processes, medical orders, agency mission and protocols and Patient’s Bill of Rights.

- Coordinates the plan of care and participates in ongoing interdisciplinary patient assessment with other clinicians, including nursing staff, physical therapists, occupational therapists, speech language pathologists, home health aides, dieticians, and medical social workers.

- Participates in the Quality Assessment and Performance Improvement (QAPI) program.

- Participates in organization sponsored inservice training.

- Completes mandatory and department competencies annually and as necessary.

Required Qualifications*

- Master’s Degree in Social Work from a school of social work accredited by the Council on Social Work Education

- One (1) year experience as a Medical Social Worker in a health care setting

- Current clinical license (LMSW) to practice social work in the state of Michigan; Licensure will be verified via LARA.

- Ability to demonstrate excellent problem solving and triage skills

- Ability to work independently in a multidisciplinary, fast-paced environment as demonstrated by interview, professional references and performance evaluations

- Demonstrated proficiency with computer skills, Microsoft Word, Excel, e-mail, and smart phones

- Excellent communication and customer service skills

- Ability to travel from point A to point B throughout the workday as assigned

PHYSICAL REQUIREMENTS:

The employee will be able to perform the assigned duties and responsibilities with or without reasonable accommodations: ability to work in a sedentary position for long periods of time, ability to communicate/ exchange information, fine motor coordination/dexterity necessary for keyboarding or typing, and may require reaching, bending, twisting, stooping, moving about, lifting of up to 35 pounds, etc., with or without the use of assistive/adaptive devices.

Work Environment:

The job requires the health care professional to work with the patient in the patient’s home. Must be able travel from point A to point B throughout the workday in all kinds of weather as assigned. Environmental factors of the patient’s home are not within the control of the Health System (i.e., whether or not there is a smoker in the home, pets, apartment complexes, home entrance, etc.).

Desired Qualifications*

- Experience or Certification in Geriatrics

- Two to three years as an MSW in a home health care setting

- Previous experience with Epic or Epic Home Health

Work Schedule

Hours:  Part time/20 hours per week
Shift/Hours/Days:  :  Specific days & hours to be determined upon hire; some evenings and weekends may be required

 

Work Locations

This position provides home visits throughout southeast Michigan.

Additional Information

Must comply with all UMHS Occupational Health requirements including annual Flu immunizations.
