Job Summary

The Little Victors League (LVL) is seeking a full-time Complex Care Navigator (RN).  LVL is a support program dedicated to children who are born with or develop complex medical conditions.  LVL’s mission is to enable these children and their families to thrive and live their best lives.  LVL provides highly organized care from prenatal diagnosis, throughout the baby’s NICU and subsequent hospital stays, to home and in the community.  LVL aims to coordinate resources for our most fragile babies and their families across the continuum of care that begins in the NICU and continues to home and community.

Navigating the healthcare system can be a daunting task. The LVL Complex Care Navigator (RN) will be a part of the outpatient team providing this care. The LVL team is comprised of inpatient and outpatient RN complex care navigators, social workers, support team professionals, with supervising physicians and specialists at C. S. Mott Children’s Hospital.

Salary range for RSAM (Role-Specific Advancement Model) RNs- $70,009.07 - $110,998.57

  • Evening Shift Differential- $2.45 per hour
  • Night Shift Differential- $3.30 per hour
  • Weekend Differential- $1.75 per hour

Responsibilities*

The LVL Complex Care Navigator (RN) will assist in coordinating the clinical activities of the multidisciplinary teams that care for LVL patients with complex care needs after discharge from the hospital and through partnership with the inpatient coordinator for subsequent hospital admissions.  The LVL Complex Care Navigator (RN) promotes and facilitates optimal care and works with a diverse group of individuals including families, physicians, nurses, donors and other health system staff through clinical, administrative, professional, educational and quality improvement activities.  The LVL Complex Care Navigator (RN) delivers evidence-based care and establishes best practices in clinical care of the complex patient.  The individual will work as a liaison between families and physicians, surgeons, consulting services, and ancillary support to organize care for each of these children and advocate for families as they navigate the healthcare system.

 

Characteristic Duties and Responsibilities:

  • Provides expert care coordination, education, consultation and advocacy.
  • Works with a primary care provider and outpatient consulting services and hospital personnel to develop an organized care plan for patients in the program
  • Acts as the primary resource for care coordination for families. Completes assessments, identifies goals and objectives for performance improvements in order to provide the best patient care, optimum customer service and improve overall satisfaction.
  • Coordinates the specific program activities for the patient population.
  • Works collaboratively with the inpatient and outpatient case managers, social workers, agencies, and families to coordinate care of the complex patient.
  • Participates in transitions from the inpatient to the outpatient setting and vice versa. Supporting patient education for the clinical diagnosis as well as educating families on community resources which are available.
  • Provide timely and service-oriented approach to telephone calls. Triage phone calls using identified algorithms in accordance with best practices adopted by the program and the American Academy of Pediatrics, and consult with other team members as necessary.
  • Provide pager coverage during the daily shift.
  • Partnering with inpatient LVL Complex Care Navigator and Outpatient Social Worker to participate in preparations for discharge from the NICU/hospital.
  • Contact insurance companies and review agencies to initiate prior authorizations for medications, supplies, clinical care and diagnostic tests.
  • Attend home/video visits when appropriate
  • Primary POC

Organizational Relationships:

Directly responsible to the NICU Clinical Nursing Director for practice and professional issues.  Will have a reporting relationship with the medical directors of the Little Victors League program.  May be responsible for directing other registered nurses, licensed practical nurses, medical assistants or nursing assistants in coordination of patient care activities.  Collaborates with nurses, physicians, social workers and other health care groups in the hospital and community.

 

Expectations of the position:

  • Demonstrated excellent written and verbal communication skills.
  • Demonstrated competency in the assessment and interventions of chronic complex medical regimens, and patient and family education and counseling.
  • Ability to interact with all members of the interdisciplinary team effectively and manage multiple tasks simultaneously.
  • Demonstrated competency with telephone triage, physical assessment and intervention.
  • Demonstrated ability to work in an autonomous fashion while utilizing critical thinking skills and appropriate, available resources.
  • Demonstrated collaborative behavior in working with peers, physician assistants, NPs, MSWs, and attending physicians.
  • Demonstrated problem solving and critical thinking skills.
  • Experience with MiChart Notifications, Word, Excel.
  • Experience in the role of a Charge Nurse or equivalent leadership experience.
  • Knowledge of UMMC nursing policies and standards of practice.

Required Qualifications*

Required Educational Requirement; applicant must meet one of the following:

Bachelor’s degree in Nursing OR

Associates degree or diploma in Nursing and a Master’s degree or higher in Nursing

  • Current licensure to practice nursing in the State of Michigan.
  • Minimum of three (3) years recent (within 5 years) experience in a pediatric ambulatory clinic.
  • Minimum of three (3) years recent (within 10 years) experience in a neonatal/pediatric inpatient hospital setting.
  • Experience with chronic complex conditions in the neonatal/pediatric population.
  • A cover letter is required for consideration for this position and should be attached to your resume.  The cover letter should:  1) Describe your background, qualifications and why you believe you would be a good fit for this position; 2) Outline your specific experience with chronic complex pediatric population; 3) Specify your prior experience with ambulatory care.  The cover letter should not exceed one page in length.
