Responsibilities*

  • This Advanced Practice Provider (APP) will provide health care and specialized health services to patients and families enrolled in the ambulatory care electrophysiology/Device settings.    
  • Under physician supervision, the Advanced Practice Professional will coordinate care of clinic patients, perform health assessments including patient history/physical examination, diagnostic test, referrals, assign appropriate interventions, place orders, and follow up with the patients.
  • The role also includes interpretation, documentation, billing of scheduled and alert remote monitoring data for pacemaker and implantable cardioverter defibrillator patients.
  • The Advanced Practice Professional will maintain accurate documentation regarding patient status, evaluations, and treatments to physicians, the RN/Clinical Team; streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external UMHS team members and other ambulatory care providers
  • Device program Responsibilities would include: 
  • Will also provide clinical expertise and direction to support patient medication management.
  • Practice will be in partnership with attending EP cardiology faculty and fellows.
  • Will collaborate with other care providers/consultants to coordinate and communicate clinical care.
  • Coordination of care between clinical care coordinators, physicians and other members of the health care continuum. 
  • Participate in quality assurance activities, process improvement projects (LEAN), and facilitate growth.
  • Participate in the development of policies, procedures and protocols including standards of care.
  • Participate in research projects. 
  • Participate in monthly meetings and case presentation related to patient population.
  • Perform other related duties as required to assure patient safety and satisfaction
  • Participate in the development and presentation of educational experiences for patients, families and health care professionals including workshops, lectures, and in-service programs.
  • Utilize Service Excellence and Patient Family Centered Care in daily practice
  • Abide by the standards of care and standards of practice established by University of Michigan Health System 
  • Practice within the regulations of the Michigan Public Health Code 
  • Provide leadership and demonstrate strong skills of coordinating a multidisciplinary approach to support the patients. 
  • Serve as a preceptor for APP students

Required Qualifications*

Nurse Practitioner Requirements: 

  • Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
  • Current specialty certification to practice as an Adult-Gerontology Acute Care Nurse Practitioner, Adult Primary Care Nurse Practitioner 
  • Current specialty certification to practice as a Family Practice Nurse Practitioner will be considered if the applicant has five or more years of APP experience in Electrophysiology with five or more years of APP experience in EP device interrogation
  • Graduate of an accredited Nurse Practitioner program.
  • Minimum of three years of recent (within the last five years) APP experience in adult cardiology.
  • Current DEA registration required
  • AHA BLS and ACLS required

Physician Assistant Requirements:

  • Current license to practice as a Physician Assistant in the State of Michigan.
  • Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
  • Certification by the National Commission on Certification for Physician Assistants (NCCPA).  
  • Minimum of three years of recent (within the last five years) APP experience in adult cardiology.
  • State of Michigan Controlled Substance License Required  
  • Current DEA registration required
  • AHA BLS and ACLS required

Desired Qualifications*

  • One year of recent (within the last five years) APP experience caring exclusively for Electrophysiology Adult patients.
  • One year of recent (within the last five years) APP ambulatory care experience.  
  • One or more years of recent (within the last five years) APP experience with interrogating, interpreting and programming CIEDs (pacemakers, defibrillators), including home monitoring follow-up.
  • One or more years of recent (withing the last five years) APP experience reviewing echo reports. nuclear medicine studies, PFT, CXR, MRI, and rhythm monitoring (Holter?s, ZIO patches, etc), telemetry and basic ECG interpretation.
  • Recent experience providing care to patients with arrhythmias, CIEDs, syncope, heart failure and acute coronary syndromes.
  • Demonstrated knowledge of adult cardiac physiology.
  • Demonstrated ability to complete history and physical examination; order appropriate diagnostic studies; synthesize information and make informed clinical decisions; develop appropriate care plans; responsive to patient and family preferences and needs.
  • Demonstrated ability to collaborate effectively in a multidisciplinary team as evidence by previous experience within the last 2 years.
  • Must demonstrate a high level of independent problem solving and decision-making abilities. 
  • Demonstrates ability to prioritize assignments and manage busy work environment
  • Demonstrated excellent, professional verbal and written communication skills.
  • Ability to demonstrate good computer skills including computer documentation and order entry.
  • Demonstrates clinical leadership skills: Chaired or participated a committee, actively involved in professional organizations, developed protocols, etc.

     

Work Schedule

?       Hours/Week: 40 hours/week

?       Location: Cardiovascular Center Clinics or other off-site locations as necessary
Ambulatory Care Work: 80% patient care, 20% administrative time.

This position is covered under the collective bargaining agreement between the U-M and the United Physician Assistants of Michigan Medicine, which contains and settles all matters with respect to wages, benefits, hours and other terms and conditions of employment.
