Job Summary

To provide comprehensive care management services to Michigan Medicine patients with complex medical and psychosocial needs. Works to engage patients with a focus on improving patient experience, strengthening care teams, improving health and reducing costs. Care management of complex patients involves advocacy, collaboration, and organization of patient care activities to facilitate optimal delivery of health care services

Responsibilities*

  •  Ensure timely and effective communication between patients and care team members, especially during transitions between inpatient, subacute, and community-based settings.
  • Assist patients achieve optimum health by coordinating services, supporting patient needs, and collaborating with families, providers, and community agencies; assist with calls, benefits applications, and advocacy around resources. Ensuring patients have access to health insurance coverage and benefits 
  • Evaluate patients  mental, physical, behavioral, social, cultural, functional, and environmental factors that may affect access to healthcare.
  • Organize and coordinate care across different disciplines, locations, and levels of care, including facilitating appointment scheduling and arranging non-emergency transportation.
  • Help identify and advocate for opportunities to enhance quality and effectiveness of care. Clinical and Behavioral Health related.
  • Serve as a resource and internal consultant regarding care management and community resources to health system staff and departments.
  • Maintain all required documentation of activities in MiChart.
  • Attend medical and specialty appointments with patients when appropriate. Complete home visits when indicated and attend community appointments when indicated.
  • Work closely with PCPs and other healthcare team members, sharing critical patient information and updates.
  • Attend daily huddles and staff/community meetings as required.
  • Participate in Quality Improvement activities as assigned.
     

Required Qualifications*

  • Bachelor's Degree in Nursing, Social Work, Psychology or other related field.
  • Two or more years of progressively responsible experience providing care/case management services

Desired Qualifications*

  • Masters level degree in Social Work, Nursing, Psychology or other related field.
  • Experience working with mental health/substance use disorder
  • Experience working in a large, academic healthcare setting
  • Current Case Management certification or the ability to become certified within one year of employment.

Modes of Work

Union Affiliation

This position is covered under the collective bargaining agreement between the U-M and the United Michigan Medicine Allied Professionals (UMMAP), which contains and settles all matters with respect to wages, benefits, hours, and other terms and conditions of employment.

Background Screening
