Job Summary

The Admissions Triage Coordinator (ATC) plays a critical role at the intersection of clinical care and patient access, serving as a primary gateway for patients entering inpatient rehabilitation services at the U-M Health joint venture unit at Chelsea Hospital, as well as the pediatric program at Mott Children?s Hospital.

This highly collaborative position partners with physicians, care managers, payers, and interdisciplinary teams to identify appropriate candidates, coordinate timely admissions, and ensure a smooth, safe transition of care into the rehabilitation setting.

Using clinical expertise and strong analytical skills, this role evaluates cases against regulatory and payer criteria, coordinates clinical details to ensure safe patient transitions, facilitates insurance authorizations, and proactively addresses barriers to optimize patient flow and program census.

Responsibilities*

Bed Planning and Pre-admission Processing:

? Coordinates the placement of rehab admissions in accordance with site specific clinical, regulatory and safety standards

? Maintains a potential admission queue and wait list for inpatient rehabilitation

? Educates patients/family, referral sources, payers and case managers on IRF programming and services, insurance and admission criteria

? Communicates daily with physician consult teams to identify inpatient rehab candidates, projected admission dates and patient prioritization

? Projects occupancy by continuously reviewing scheduled admissions and discharges

? Identifies, reports, and resolves actual and potential admissions delays

? Reviews patient cases and applies clinical knowledge to identify key clinical and regulatory considerations/barriers

? Presents patient case details in multidisciplinary planning huddles to facilitate unit-wide readiness for each patient admission

? Completes chart documentation review and compiles pertinent clinical information required for the pre-admission screening.

Insurance Coordination:

? Evaluates specific clinical information against established CMS and InterQual criteria for completion of preadmission medical necessity reviews

? Initiates insurance prior authorization processes for inpatient rehab stays

? Coordinates peer-to-peer reviews between insurance company medical directors and rehabilitation physicians when necessary

? Facilitates internal physician-to-physician communication and secondary review by Medical Director for any cases in which admittance criteria are in question

? Completes weekly continued stay reviews and re-certifications based on individual insurance requirements

? Maintains thorough and clear documentation of all payer contacts and reviews. Clearly documents all authorizations, actions, contacts, outcomes and interventions to assure appropriate payment of claims

Other Duties:

? Collects, documents, and analyzes a range of data to evaluate referral and admission trends and support program performance optimization

? Monitors, reviews, and queries pre- and post-admission documentation to assure compliance requirements are met

? Participates in the development of standard work, process improvement, and policy and procedure development

? Coordinates facility tours and participates in program marketing

Required Qualifications*

  • Bachelor's degree in nursing or related allied health discipline (such as respiratory therapy, social work, occupational or physical therapy) or an equivalent combination of education and directly related experience.
  • Current applicable state licensure.
  • Minimum three years of experience in healthcare administration, clinical care or care management.
  • Excellent verbal and written communication, decision making, accountability, analysis, and time management.
  • Experience with Microsoft Excel, Outlook required.

Desired Qualifications*

  • Experience with Epic EMR preferred.
  • Previous work experience in the inpatient rehabilitation setting preferred.

Modes of Work

Positions that are eligible for hybrid or mobile/remote work mode are at the discretion of the hiring department. Work agreements are reviewed annually at a minimum and are subject to change at any time, and for any reason, throughout the course of employment. Learn more about the work modes.

Work Schedule

Work Hours:
? Full-time, schedule to be determined; Monday-Friday (hours falling between 7am-4pm) but will include weekend coverage.
? This is a hybrid position which includes both remote and onsite work at University Hospital.

Background Screening

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings.  Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.
