This Social Worker would provide psychosocial support, care management, and care coordination for adult patients across the outpatient neurorehabilitation sites (Briarwood Building 04/MedRehab, Brighton Center for Specialty Care, and Canton Health Center) as well as the Physical Medicine & Rehabilitation (PM&R) Clinic at the Burlington Building.
