Job Summary

Clinical Pharmacist Specialist Ambulatory Care- Complex Outpatient Antimicrobial Therapy Program

Provide ambulatory clinical infectious disease pharmacy services through ambulatory care complex outpatient antimicrobial therapy affiliated with University of Michigan Hospital and Health Centers.

Individual must exhibit a professional and positive image when interacting with patients, faculty and staff.  Individual must also adhere to a high standard of personal and professional conduct; possess excellent interpersonal and communication skills and problem-solving skills; demonstrated ability to interact with a diverse population. Candidates must be able to work effectively with a multidisciplinary team, handling multiple responsibilities in a fast-paced environment.

Responsibilities*

  • Assist with appropriate antibiotic selection at the time of discharge. 
  • Provide early post-discharge follow-up by way of clinical pharmacy visits via telehealth and in-person when needed.
  • Assist in the development and implementation of strategies to minimize hospital admissions and emergency department burden for patients with targeted infections. 
  • Participate in the training of General Pharmacy Practice Residents, infection disease pharmacy fellow, and Ambulatory Care Specialty Residents, as well as Pharmacy Students.
  • Conduct or participate in drug use projects, cost reduction/management initiatives, and quality improvement activities consistent with the Health System?s needs and objectives.
  • Individual or collaborative scholarly/research activity is encouraged.
     

Required Qualifications*

  • A Doctor of Pharmacy degree or an equivalent combination of education and experience.
  • A certificate of completion from a General Practice residency is necessary.
  • A certificate of completion from a specialty residency in infectious diseases is preferred.  
  • Experience in clinical pharmacy practice and antimicrobial stewardship is necessary. 
  • Licensure as a Pharmacist by the State of Michigan is necessary.
  • Possession of a state controlled substance license is necessary.
  • Demonstrated ability to collaborate clinical care ideas across multiple patient care services and to communicate effectively as a patient advocate.
  • Demonstrates a commitment to carrying out professional responsibilities, adherence to ethical principles, and sensitivity to diversity with a commitment to service excellence and Patient Family Centered Care.

Supervision Exercised

Functional supervision is exercised over Pharmacy Residents and Students.
 

Background Screening

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings.  Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.
