Responsibilities*
- This Advanced Practice Provider (APP) will provide health care and specialized health services to patients and families enrolled in the Heart Failure Program in Cardiovascular Medicine.
- Under physician supervision, the APP will coordinate care of this patient population before and after hospitalization
- Independently evaluate patients in the clinic for initial and return visits as well as pre & post hospitalization appointments.
- Independently evaluate and treat patients in the IV diuresis clinic
- Order/interpret testing and lab results, modifying plan of care to address results.
- Provide clinical expertise and direction to support patient medication management
- Collaborate with other care providers/consultants to coordinate and communicate clinical care
- Maintain accurate documentation regarding patient status, evaluations, and treatments for communication with physicians and APPs, and the heart failure Tele-management RN/Clinical Team; including streamline the patient transfer process between levels of care.
- Communicating specific patient needs to internal and external UMHS team members and other ambulatory care providers
- Participate in quality assurance activities including Lean Projects within the Heart Failure Program in Cardiovascular Medicine
- Perform other related duties as required to assure patient safety and satisfaction
- Participate in the development and presentation of educational experiences for patients, families, and health care professionals.
- Utilize Service Excellence and Patient Family Centered Care in daily practice
- Abide by the standards of care and standards of practice established by University of Michigan Health System
- Collaborate with Heart Failure physicians to provide care across the continuum
- Practice within the regulations of the Michigan Public Health Code
Required Qualifications*
For Nurse Practitioners (NP):
- Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan.
- Current specialty certification to practice as an Adult-Gerontology Acute Care NP, Adult Primary Care NP, or Acute Care NP
- Graduate of an accredited Nurse Practitioner program.
- Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN).
- Current AHA BLS and ACLS certifications
- Current DEA registration required
- A minimum of three years APP experience, in the last 5 years, working exclusively with Adult Cardiology patients.
For Physician Assistants (PA):
- Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP.
- Certification by the National Commission on Certification for Physician Assistants (NCCPA).
- Current license to practice as a Physician Assistant in the State of Michigan.
- State of Michigan Controlled Substance License Required
- Current AHA BLS and ACLS certifications
- Current DEA registration required
- A minimum of three years APP experience, in the last 5 years, working exclusively with Adult Cardiology patients.

Desired Qualifications*
- 3-5 years APP experience, within the last 5 years, evaluating and treating Heart Failure or Heart Transplant patients in an inpatient or clinic setting
- 1 year previous APP experience in an adult cardiology clinic
- APP experience reviewing cardiac catheterization, echo reports, nuclear medicine studies, rhythm monitoring (Holter?s, ZIO patches, telemetry, etc.), and ECG interpretation.
- APP experience with medical management for adult heart failure patients, including but not limited to IV diuresis
- Certification in one or more areas of cardiology: CCKE, HF-Cert, CHFN-K, ABCL, or CHC
- Demonstrated knowledge of adult cardiac physiology.
- Demonstrated ability to complete history and physical examination; order appropriate diagnostic studies; synthesize information and make informed clinical decisions; develop appropriate care plans; responsive to patient and family preferences and needs.
- Must demonstrate a high level of multi-tasking, critical thinking, and independent problem solving with decision making abilities.
- Demonstrated ability to collaborate effectively in a multidisciplinary team as evidence by previous experience within the last 2 years.
- Demonstrated excellent, professional verbal and written communication skills

Modes of Work

Work Schedule
Hours/Week: 40 hours
Shift/Days/Hours: Day shift; specific days and hours to be determined. May include evenings and weekends.
Location: Cardiovascular Center Clinics or other locations as needed.
Union Affiliation
This position is covered under the collective bargaining agreement between the U-M and the United Physician Assistants of Michigan Medicine (UPAMM), which contains and settles all matters with respect to wages, benefits, hours, and other terms and conditions of employment.
