Responsibilities*

  • Work in collaboration and provide care coordination with the ALS multidisciplinary team as well as supporting other needs within the neuromuscular and neurology division. 
  • Possessing clinical knowledge regarding chronic care management of patients with ALS and other neuromuscular disorders 
  • Tele-health nursing to include: triaging calls from patients and families, clinicians, and other healthcare team. Nurse led education visits for new diagnosis and ongoing treatment plan care coordination included in role 
  • Assist in clinic with multidisciplinary visits to support and coordinate care with patients and families 
  • Collaborate with other health team members and associated resource persons to ensure continuity of care
  • Assist in the development and implementation of competencies, protocols, and patient education tools to improve patient care.
  • Participation in Taubman Neurology clinic team meetings to continually improve clinic processes.
  • Candidate must be able to function with a minimum of supervision and be highly motivated to provide excellent care and patient advocacy.
     

Required Qualifications*

  • Current license to practice Nursing in the State of Michigan
  • Bachelors degree in Nursing OR Associates degree/diploma in Nursing along with a Masters degree in Nursing
  • Minimum 3 years of RN experience (within the past 5 years) performing direct patient care with adult patients with neurological conditions (preference to neuromuscular population given)

Modes of Work

Work Schedule

  • Hours: 40 hours per week
  • Shift: Days: Monday- Friday; days and hours variable; flexibility in scheduling required
  • Location: Taubman Center & Med Inn, may support needs across the neurology service line

This position is covered under the collective bargaining agreement between the U-M and the Michigan Nurses Association and the U-M Professional Nurse Council union, which contains and settles all matters with respect to wages, benefits, hours and other terms and conditions of employment.
